Inclusion/Exclusion Criteria Inclusion Criteria • Age > 21 years of age • Left ventricular ejection fraction ≤ 40% • New York Heart Association class II-IV symptoms • Hospitalization, ED visit, or outpatient therapy for ADHF within 6 months Exclusion criteria • Serum creatinine > 2.5 mg/dl • Inoperable aortic valve disease • Life expectancy
