A New Wrinkle<br />
In Facial Treatments<br />

Plastic surgery is an exciting and<br />
constantly changing field of medicine.<br />
In recent years advances in aesthetic (or<br />
cosmetic) plastic surgery have been<br />
geared toward less invasive techniques.<br />
Endoscopic or limited-incision surgeries,<br />
Botox treatments and a variety of laser<br />
resurfacing procedures are all good<br />
examples of this newer, more subtle approach.<br />
Along these lines, the most exciting developments to have<br />
come along in 2004 are a new class of fillers designed to safely<br />
minimize facial wrinkles.<br />
In previous years, individuals wishing to diminish facial<br />
wrinkles with injectable fillers relied mostly on traditional<br />
collagen. Traditional collagen is derived from animals (cow),<br />
requires an allergy test and lasts approximately three months.<br />
Human (cadaver-derived) collagen has also been used, and<br />
may last longer. In <strong>Jan</strong>uary, 2004, the U.S. Food and Drug<br />
Administration approved the use of a new type of injectable<br />
filler created from non-animal based hyaluronic acid.<br />
Hyaluronic acid is a natural substance that already exists in<br />
the human body and is used to provide volume and fullness<br />
to the skin. Known as Restylane and Perlane, these new products<br />
are safe and natural, and work well to correct moderate<br />
to severe wrinkles and folds.<br />
Restylane lasts approximately six months (twice as long<br />
as collagen), so treatments are typically scheduled twice a<br />
year. Restyane is injected directly into the skin in tiny<br />
amounts with an ultrafine needle. Results are practically<br />
instantaneous. To optimize comfort during the short procedure,<br />
many physicians anesthetize the treatment area or prescribe a<br />
topical anesthetic to be applied one hour before.<br />
Patients often will combine Restylane treatments with<br />
Botox injections. Botox blocks nerve transmission to relax the<br />
underlying muscles, and is especially helpful in the upper face<br />
(forehead and frown lines, as well as crow’s feet). Restylane is<br />
most useful in the lower face for lip enhancement and<br />
nasolabial folds (lines extending from the nose to the corners<br />
of the mouth). <strong>The</strong>se “lunch hour” procedures require no<br />
pretesting, but a few precautions are recommended before<br />
treatment. Aspirin and non-steroidal anti-inflammatory medications<br />
should be avoided for one week prior to treatment to<br />
minimize any risk of bruising. Post-treatment application of<br />
ice is also helpful.<br />
<strong>The</strong> new injectable fillers are a safe and effective way to<br />
enhance a more youthful appearance. However, in some cases<br />
more extensive work may be needed to achieve the results<br />
an individual desires. In these situations, more traditional<br />
techniques such as facelifts, browlifts and blepharopasties<br />
may be indicated.<br />
<strong>The</strong> first step in making an educated determination about<br />
the most appropriate options should be a visit to the plastic<br />
surgeon’s office. A private consultation with a board-certified<br />
plastic surgeon will clarify which procedures can be expected<br />
to yield optimal results. <strong>The</strong> American Society for Aesthetic<br />
