A New Wrinkle
In Facial Treatments
By Dr. Pamela Rosen
Plastic surgery is an exciting and
constantly changing field of medicine.
In recent years advances in aesthetic (or
cosmetic) plastic surgery have been
geared toward less invasive techniques.
Endoscopic or limited-incision surgeries,
Botox treatments and a variety of laser
resurfacing procedures are all good
examples of this newer, more subtle approach.
Along these lines, the most exciting developments to have
come along in 2004 are a new class of fillers designed to safely
minimize facial wrinkles.
In previous years, individuals wishing to diminish facial
wrinkles with injectable fillers relied mostly on traditional
collagen. Traditional collagen is derived from animals (cow),
requires an allergy test and lasts approximately three months.
Human (cadaver-derived) collagen has also been used, and
may last longer. In January, 2004, the U.S. Food and Drug
Administration approved the use of a new type of injectable
filler created from non-animal based hyaluronic acid.
Hyaluronic acid is a natural substance that already exists in
the human body and is used to provide volume and fullness
to the skin. Known as Restylane and Perlane, these new products
are safe and natural, and work well to correct moderate
to severe wrinkles and folds.
Restylane lasts approximately six months (twice as long
as collagen), so treatments are typically scheduled twice a
year. Restyane is injected directly into the skin in tiny
amounts with an ultrafine needle. Results are practically
instantaneous. To optimize comfort during the short procedure,
many physicians anesthetize the treatment area or prescribe a
topical anesthetic to be applied one hour before.
Patients often will combine Restylane treatments with
Botox injections. Botox blocks nerve transmission to relax the
underlying muscles, and is especially helpful in the upper face
(forehead and frown lines, as well as crow’s feet). Restylane is
most useful in the lower face for lip enhancement and
nasolabial folds (lines extending from the nose to the corners
of the mouth). These “lunch hour” procedures require no
pretesting, but a few precautions are recommended before
treatment. Aspirin and non-steroidal anti-inflammatory medications
should be avoided for one week prior to treatment to
minimize any risk of bruising. Post-treatment application of
ice is also helpful.
The new injectable fillers are a safe and effective way to
enhance a more youthful appearance. However, in some cases
more extensive work may be needed to achieve the results
an individual desires. In these situations, more traditional
techniques such as facelifts, browlifts and blepharopasties
may be indicated.
The first step in making an educated determination about
the most appropriate options should be a visit to the plastic
surgeon’s office. A private consultation with a board-certified
plastic surgeon will clarify which procedures can be expected
to yield optimal results. The American Society for Aesthetic
Plastic Surgery (800-364-2147 or www. Asaps.org.) is a good
resource for finding qualified plastic surgeons. P ●
