Registration/Payment Information Online Catalog Advanced Registration Payment Check A copy of this catalog is available on our website at www.casat.unr.edu. Click on CASAT Training Opportunities. (The catalog is a PDF file. You will need Acrobat Reader which is a free download from www.adobe.com/reader. The website includes a copy of the registration form.) Advanced registration is requested. In the event that a workshop reaches capacity, registration may be closed prior to the day of the workshop. Please pay all fees in advance. We accept payment by check, money order, purchase order, cash, or credit cards. Please do not send cash through the mail. Certificates will not be processed until payment has been received. Registration/Payment Cont. Confirmation Participants will receive written confirmation notice, provided the registration form is received at least 10 days prior to the workshop. Please review confirmation notice for any possible changes in location. • y Refunds will be made ONLY if the cancellation is received at least three working days prior to the workshop. • y Social Security or tax ID number is required for refunds paid for by check. Scholarships are primarily for persons with extremely limited resources. Scholarship applications must be made 30 days in advance of the workshop to be considered. Applicants can only receive a scholarship for one class per catalog. Application Process To apply, send the following information with your workshop registration: 1. A list of training scholarships received in the past year. 2. The reasons why you need the scholarship assistance.
