who received the first dose of Hib vaccine on or after 15 months of<br />

age are required to have only one dose. Persons who have passed<br />

their fifth birthday are not required to be vaccinated against Hib.<br />

Hepatitis B 3 One dose by age three months, second dose by age five months and a<br />

third dose by age 19 months. Persons born on or after July 1, 1994<br />

are required to receive the hepatitis B vaccine. The last dose of<br />

hepatitis B series shall not be administered prior to 24 weeks of age.<br />

Varicella 1 One dose of varicella vaccine - on or after 12 months of age and<br />

before age 19 months - is required for children born on or after<br />

April 1, 2001. However, an individual with laboratory tests showing<br />

immunity or a history of chickenpox documented by a health care<br />

provider, parent, guardian or person in loco parentis shall not be<br />

required to receive varicella vaccine. Documentation of prior history<br />

of chickenpox disease must be on or attached to the lifetime<br />

immunization card or certificate of immunization. Ask your health<br />

care provider for more information.<br />

A physician, health clinic or local health department administering required vaccines must give a Certificate of<br />

Immunization to the person who presented the child for immunization. The certificate/record must include the<br />

following information: a) name, sex and date of birth; b) name and address of parent or guardian; c) number of<br />

vaccine doses given; d) date vaccines were given (month and year is acceptable for out-of-state transfer students);<br />

and e) name and location of provider.<br />

No child shall attend a school (PreK - 12) unless a certificate of immunization indicating that the child has received<br />

the immunizations required by G.S. 130A-152 is presented to the school or facility. The parent, guardian, or<br />

responsible person must present a certificate of immunization on the child's first day of attendance to the principal or<br />

designee of the school. If a certificate of immunization is not presented on the first day, the principal shall present a<br />

notice of deficiency to the parent, guardian or responsible person. The parent, guardian or responsible person shall<br />

have 30 calendar days from the first day of attendance to obtain the required immunization for the child. If the<br />

administration of vaccine in a series of doses given at medically approved intervals requires a period in excess of 30<br />

calendar days, additional days upon certification by a physician may be allowed to obtain the required<br />

immunization. Upon termination of 30 calendar days or the extended period, the principal shall not permit the child<br />

to attend the school or facility unless the required immunization has been obtained.<br />

*Tdap IMMUNIZATION REQUIREMENT FOR STUDENTS ENTERING 6 TH GRADE<br />

All individuals attending public school who are entering the 6th grade must have the Tdap vaccine, if five years or<br />

more have passed since the last dose of tetanus/diphtheria toxoid. An official vaccination record must be presented<br />

to the middle school nurse. The NC Immunization Law and the suspension guidelines, as stated above, will be<br />

followed. If your student needs this vaccination, call the local health department or your child’s healthcare provider<br />

for an appointment. Contact the school nurse for more information.<br />

WHEN IMMUNIZATIONS ARE “IN PROCESS”<br />

Children who have begun a series of immunizations, but have not completed the series due to the need to meet<br />

minimum intervals between doses, are considered in process and may attend school.<br />

EXEMPTIONS<br />

There are two exemptions to required immunizations.<br />

1. Medical Exemptions - An exemption is permitted for medical reasons when a physician determines that an<br />

immunization is or may be harmful to a student for a specific reason. Valid medical exemptions must be written<br />

and signed by a physician licensed to practice medicine in North Carolina. The medical exemption must<br />

correspond to those medical contraindications specified in the N.C. Immunization Rules or an approved Rules'<br />

exception approved by the State Health Director. These physician statements must be maintained in the student's<br />

permanent record and at minimum must indicate the following - basis of the exemption; specific vaccine(s) the<br />

child should not receive; and length of time the exemption will apply for the child. Medical Exemption Statement<br />

(DHHS 3987).<br />
