Page | 108Upward Mobility Nursing ProgramThe Upward Mobility Nursing Program leads to the Associate of Applied Science degree and prepares graduates forthe National Council Licensure Examination (NCLEX-RN) to become a Registered Nurse (RN).The program provides an opportunity for the LVN/LPN to advance to the RN level with minimal duplication of basiceducation and skills courses.Courses in the program encourage the acquisition of the knowledge, skills and values that enable graduates tosucceed in the healthcare field.Admission RequirementsIn addition to applying for admission to <strong>Lamar</strong> <strong>State</strong> <strong>College</strong>-<strong>Port</strong> <strong>Arthur</strong>, students must apply for admission to theUpward Mobility Nursing Program. Applicants must be currently Licensed Vocational Nurses or Licensed PracticalNurses. Requirements and application forms are available from the Allied Health Department.The number of students admitted to any class is governed by the requirements of the Board of Nursing. The UpwardMobility Nursing Standards Committee selects the best qualified applicants. The Admission Criteria Rating Form wasdeveloped to aid the objective selection of the class. Applicants are ranked according to their score on the AdmissionCriteria Rating Form.In any given admission period, those applicants having the highest scores are the ones admitted to the program.<strong>Lamar</strong> <strong>State</strong> <strong>College</strong>-<strong>Port</strong> <strong>Arthur</strong> is an open access institution. All available slots are filled with the most qualifiedapplicants available in any given admission period.Successful completion of the Upward Mobility Nursing Program does NOT guarantee that a graduate may take theNCLEX-RN. Students who do not satisfactorily complete the program exit exam will not be allowed to graduate.Eligibility for LicensureThe Texas Board of Nursing identified certain circumstances that may render a potential candidate ineligible forlicensure as a nurse in the <strong>State</strong> of Texas. The board provides individuals with an opportunity to petition for aDeclaratory Order as to their eligibility in accordance with Article 301.257 of the Nursing Practice Act. Information isavailable on the TBON website: www.bon.state.tx.us.A Petition for Declaratory Order must be submitted by the nursing candidate if the candidate answers “yes” to any ofthe following questions:1. For any criminal offense including those pending appeal (excluding Class C misdemeanor traffic violations), haveyou:– been convicted of a misdemeanor?– been convicted of a felony?– pled “nolo contendere,” no contest or guilty?– received deferred adjudication?– been placed on community supervision or court-ordered probation, whether or not you were judged guilty?– been sentenced to serve jail or prison time or court-ordered confinement?– been granted pre-trial diversion?– been arrested or have any pending criminal charges?– been cited or charged with any violation of the law?– been the subject of a court martial, Article 15 violation or received any form of military judgment/punishment/action?2. Within the past five (5) years, have you been addicted to and/or treated the use of alcohol or any other drug?6. Within the past five (5) years, have you been diagnosed with, treated for or hospitalized for schizophrenia and/orpsychotic disorder, bi-polar disorder, paranoid personality disorder, anti-social personality disorder or borderlinepersonality disorder.
