3<strong>Remembrance</strong> <strong>Tribute</strong>sMemorial items can help bereaved families and friends through a very sensitive anddistressing period, giving lasting memories of a loved one. They are often a visualcomfort to those visiting the cemetery.Headstones are not available through us, you will need to contact a licensedmemorial mason who can work in our cemeteries.The pages that follow contain information about each individual remembrance tributeand in which cemeteries they are available. 5Animal granite seat plaqueThese impressive granite features can be found at Carmountside Cemetery andCrematorium in the;• 'Butterfly Dreams' babies and children's burial garden; and• 'Sweet Dreams' babies and children’s burial garden.Both babies and children's burial areas offer a peaceful setting within beautifullycreated gardens.Each seat will be shared at most by two families. The plaques are available with aninscription (message) of your choice in gold lettering.The location of the seat is at our discretion. The lease for a plaque is ten years, butyou can buy another lease after this time.If you choose not to, we will remove the plaque from display and suitably dispose ofit.Please contact us to place an order or for more information

6Arboria PlaqueThese attractively designed timber plaques can be found in the ‘Book of<strong>Remembrance</strong> Room’ at Carmountside Cemetery and Crematorium.There are four types of plaques available.• Inscription (message) only.• Inscription with engraved motif.• Inscription with engraved coloured motif.• Inscription with ceramic portrait.These plaques can hold up to five lines of text and are suitable for up to two nameson the plaque.The lease for a plaque is ten years, but you can renew it after this time.If you choose not to we will remove the plaque from display and suitably dispose of it.Family Arboria PlaqueThese attractively designed family timber plaques can be found in the 'Book of<strong>Remembrance</strong> Room' at Carmountside Cemetery and Crematorium.There are four types of plaques available.• Inscription (message) only.• Inscription with engraved motif.• Inscription with engraved coloured motif.• Inscription with ceramic portrait.These plaques can hold up to 11 lines of text and are suitable for up to two or morenames on the plaque.The lease is for ten years, but you can renew it after thistime.If you choose not to we will remove the plaque fromdisplay and suitably dispose of it.Please contact us to place an order or for moreinformation on the above items please contact theBereavement Care Service office on 01782235050.

7Babies Book or <strong>Remembrance</strong>A special way to remember a baby or child is to have an entry in the Babies Book of<strong>Remembrance</strong>.This book is on permanent display in the Book of <strong>Remembrance</strong> Room atCarmountside Cemetery and Crematorium. (Entrance to the left of the Chapel).It can be used to remember any date you wish.Each entry (hand written in traditional style) can be either:• two lines;• five lines; or• eight lines of text.Entries can't be more than 32 letters per line and the first line must be used for thename only.If your entry is more than five lines, a hand painted motif (such as a flower orphotograph) can be added.Each day of the year is displayed on a single page of the book. Your entry will bedisplayed on your chosen date.If you want to view your entry on any other day, please contact us to make anappointment. 9Book of <strong>Remembrance</strong>An entry in the Book of <strong>Remembrance</strong> is a special way to remember a loved one.The books are on permanent display in the Book of <strong>Remembrance</strong> Room atCarmountside Cemetery and Crematorium. The Entrance is to the left of the Chapel.It can be used to remember any date you wish.Each entry (hand written in traditional style) can be either:• two lines;• five lines; or• eight lines of text.Entries can’t be more than 32 letters per line, and the first line must be used for thename only.If your entry is more than five lines long a hand painted motif, such as a flower orshield, can be added at an additional cost.You can reserve space below your entry for future use.Each day of the year is displayed on a single page of the book. Your entry will bedisplayed on your chosen date.If you would like to view your entry at any other time, we can arrange to turn thepage. Otherwise it can be viewed on the Electronic Book of <strong>Remembrance</strong>.Entries are displayed in two separate books:• from 1 January to 30 June; and• from 1 July to 31 December.We add new entries when the book is not on display.All entries are at our discretion.Please contact us to place an order or for moreinformation

10Boutonierre plaqueThese attractive re-constituted granite plaques are located in the West Cloister in theGarden Of <strong>Remembrance</strong> at Carmountside Cemetery and Crematorium and also atBurslem Cemetery.There are three types of plaque available;• Text only;• Text and motif; or• Text and photograph.Each plaque comes with a posy holder at the side for flowers.The lease for a plaque is 5 years and can be renewed after this time.If you decide not to renew the lease we will remove the plaque from display andsuitably dispose of it.Please contact us to place an order or for more information

11Butterfly DreamsOctagonal seat plaqueThis can be found in the Butterfly Dreams babies and children’s burial garden atCarmountside Cemetery and Crematorium.We have recently installed a communal granite seat, octagonal in shape which ismade up of eight smaller seats with four inscribed plaques on each seat.You can lease one or more of the plaques on the seat.Each plaque will display an inscription (message) of your choice in gold lettering.The lease for a plaque is 5 years and can be renewed after this time.If you choose not to renew the lease the plaque will be removed from display andsuitably dispose of it.Please contact us for more information or to place an order

12Buxton granite seat plaqueThese striking granite features can be found at Carmountside Cemetery andCrematorium and throughout our city cemeteries except Trentham Cemetery.They are also available in the Babies and Children’s burial areas at Carmountside.Each polished granite seat is four feet long and has two plaques. You can lease oneor both plaques on the seat. The plaque(s) are engraved with an inscription(message) of your choice in gold lettering.The location of the seat is at our discretion. You will need to agree the position ofyour seat with us before it is ordered. We will arrange a meeting to discuss this priorto application.The lease for a plaque is ten years, but you can renew it after the initial lease has runout. 13Communal granite book plaqueThis can be found in Carmountside Cemetery and Crematorium, within the ButterflyDreams Babies and Children’s Burial Garden.The impressive granite book is made up of 60 granite plaques set into the pedestal,and each page of the book.Plaques are available with an inscription (message) of your choice in gold lettering.Butterfly Dreams is a beautifully designed garden where a magnificent ‘children andbutterfly’ granite statue is the central feature. It brings a peaceful feel to the gardenand makes it a nice and attractive place to visit.The lease for a plaque is for five years, but you can choose to buy another leaseafter this time. 14Communal seat plaqueThis striking feature is a focal point of the Victorian Shelter situated in the Garden Of<strong>Remembrance</strong> at Carmountside Cemetery and Crematorium.We have also recently installed a communal seat at Burslem cemetery on the site ofthe former Burslem chapel.The seat is hexagonal in shape which is made up of six smaller seats.Each seat will have two inscribed plaques.You can lease one or both of the plaques on the seat.Each plaque will display an inscription (message) of your choice in gold lettering.The lease for a plaque is 5 years and can be renewed after this time.If you choose not to renew the lease the plaque will be removed from display andsuitably disposed of.Please contact us for more information or to place an order

15Cremation remains urnsIf you wish to keep your loved ones cremation remains at home, a decorated urn is agood way to do that.A choice of Cremation Remains Urns are on display in the reception at CarmountsideCemetery and Crematorium office.We will contact you when your urn is ready to be collected.This will be in around 21 days from the date of your order.Please note that our staff do not transfer cremation remains into the urns.To place an order or for more information please contact us

16Electronic Book of<strong>Remembrance</strong>The electronic book of remembrance (with touch screen) is available to view in theBook of <strong>Remembrance</strong> Room, at Carmountside Cemetery and Crematorium.(Entrance to the left of the Chapel).If you buy an entry in either the:• Book of remembrance;or the• Babies Book of remembrance;we automatically put it onto a digital 'Touch Screen' as well.You can add to your entry by buying 'additional touch screen options', which offer upto four more screens.These can be used to display:• mementos of your choice;• personal messages;• a poem; or• family photograph.Your entry and additional screens will be displayed on the date you choose.If you would like direct access to your entry, you can buy a swipe card.All entries are at our discretion.To place an order or for more information please contact us

17Flower bed and markerA beautiful way of remembering a loved one is to have a flowerbed in their memory.Flowerbeds are available in Carmountside Cemetery and Crematorium.We take care to produce a colourful display of beds throughout the year. It makesour cemeteries a pleasant and attractive place to visit.Each flowerbed has a marker, bearing a suitable inscription (message).The lease for the flowerbed is one year - you can buy another one when this has runout. You can make an appointment with us if you would like to witness the markerbeing placed.If you choose not to renew the lease we will remove the marker from display andsuitably dispose of it.To place an order or for more information please contact us

18Heart plaqueThese striking red heart features are located in the North and South areas of theGarden of <strong>Remembrance</strong> at Carmountside Cemetery and Crematorium.The memorial is heart shaped and stands on a square pedestal base. Each heartmemorial contains five individual plaques.Each plaque is black and circular in shape and will display an inscription (message)of your choice in gold lettering.The pedestal base holds a small flower container where families can place fresh andartificial flowers beneath the plaque.The lease for the plaque is 5 years. You can renew the lease after this time. If youchoose not to, we will remove the plaque from display and suitably dispose of it.Please contact us to place an order or for more information

19Kerb VaseGranite kerb vases are available in the Garden of <strong>Remembrance</strong> at CarmountsideCemetery and Crematorium.You can find them around the Victorian Shelter in four attractively designed shrubbeds.Each shrub bed can contain a maximum of six kerb vases. The vase is made ofgranite and has a striking dark red colour and features an inscription (message) ofyour choice in gold lettering.Vases cannot be placed on the strewing area. The lease for the vase is five years.You will need to agree the position of your vase with us before it is ordered.We will arrange a meeting to discuss this prior to application.Please contact us to place an order or for more information

20Memorial seatA very popular way of remembering a loved one is to buy a memorial seat in theirmemory. They are available in all of the city's cemeteries except TrenthamCemetery.Seats are approximately five feet long and include a back rail, which will take aninscription (message) of up to 140 characters. You can buy additional rails in lateryears.Wherever possible we will place your seat where you choose. You will need to agreethe position of your seat with us before it is ordered. We will arrange a meeting todiscuss this prior to application.The fee for the first 10 year lease period includes buying the inscribed seat. Thismeans that the seat will be yours to own and maintain. You then lease the areawhere it is placed.The seat is finished in a high quality, environmentally friendly wood preservative.Five year renewals after this time are charged a lease fee only.Maintenance of the seatDuring the lease period any maintenance costs for the seat, including repairs/teak oil,remain the responsibility of the owner.If you ask us to, we will carry out any work that is needed. We do charge for this.(See our current fees and charges list).Memorial Seats remain in use for many years and are a lasting form of memorial.We will take a seat out of service when it reaches the end of its life, such as throughold age and general deterioration. 21Memory treeThis striking feature is found in the Sweet Dreams Babies and Children's BurialGarden, at Carmountside Cemetery and Crematorium.The memory tree is made up of six plaques which are positioned around the squirrel.Each plaque is circular in shape and will display an inscription (message) of yourchoice in silver lettering.The lease for the plaque is five years. You can renew the lease after this time, but ifyou choose not to, we will remove the plaque from display and suitably dispose of it.Please contact us to place an order or for more information

23Shrub and markerA beautiful way to remember a loved one is to have a shrub or rose bush in theirmemory.A wide variety are available in and at Carmountside Cemetery and Crematorium andin all of the City’s Cemeteries except Trentham Cemetery.A number of areas have been set aside to be planted with the shrubs and rose bushes.We take care to produce a colourful display throughout the year to make the areasan attractive place to visit.Each shrub/rosebush has a marker, with a suitable inscription (message). The leasefor the marker is five years, but you have the option to renew this after this time.If you don't want to, then we will remove the marker from display and suitably disposeof it.You can make an appointment with us if you would like to witness the marker beingplaced.We will maintain the shrub or rose bush and should it die within the lease period wewill replace it free of charge.Please contact us to place an order or for more information

24Rose Bush/Standard rose andmarkerA beautiful way to remember a loved one is to have a rose in their memory.Roses are available in at Carmountside Cemetery and Crematorium.A wide variety of species are available including standard roses and rose bushes.A number of areas have been set aside and planted with roses at each cemetery.We take care to produce a colourful display throughout the year to make ourcemeteries a pleasant and attractive place to visit.Each rose has a marker, with a suitable inscription (message). The lease for themarker is five years, but you can buy another one after this time. 25Sundial granite plaqueThis striking granite feature can be found in the peaceful setting of the ‘Garden of<strong>Remembrance</strong>’ at Carmountside Cemetery and Crematorium and Longton Cemetery.There are two sundials at Carmountside, one in the north of the garden and one inthe south.The sundial at Longton cemetery is closely located near the Garden of<strong>Remembrance</strong>.Each sundial has 24 granite plaques in three different sizes.The plaques are available with the inscription (message) of your choice in goldlettering.The lease for a plaque is five years, but you can buy another one after this time.If you choose not to renew the lease we will remove the plaque from display andsuitably dispose of it.Please contact us to place an order or for more information

27Teddy bear granite vasePlaqueThe Teddy Bear vase plaque has a flower container for family flowers and isavailable in the:• ‘Butterfly Dreams’ Babies and Children’s Burial Garden; and• Babies and Children’s 'Sweet Dreams' Burial Garden.The plaques are available in blue pearl or indian red granite and are inscribed with amessage of your choice. The message you choose will be in gold lettering.Both babies and children’s burial areas offer a peaceful setting within beautifullycreated gardens. You will need to agree the position of your vase with us before it isordered. We will arrange a meeting to discuss this prior to application.The lease for a granite vase plaque is 10 years. 29Topiary and markerA beautiful way to remember a baby or child is to lease a memorial topiary shrub intheir memory.A variety of impressive animal topiary shrubs are available in the ‘Butterfly Dreams’Babies and Children’s Burial Garden, at Carmountside Cemetery and Crematorium.Butterfly Dreams is a superbly designed garden where a magnificent ‘children andbutterfly’ granite statue is the central feature.It brings a peaceful feel to the garden and makes it a nice and attractive place to visit.Each shrub has a marker with the inscription (message) of your choice in goldlettering.You can watch the placing of the marker, or place it yourself if you wish.Leases are available for a period of five years.If the topiary remains in good condition at the end of the lease period, you’ll be ableto renew the lease. If you don’t want to, then we will remove the marker and disposeof it.We will look after and maintain the topiary while it's being leased.If it dies within the five year period, we will replace it free of charge.To place an order or for more information please contact us

30Train plaqueThese can be found around the fountain in the Babies and Children’s Garden atCarmountside Cemetery and Crematorium.They can be inscribed with either of the following pictures:• teddy;• doll; or• soldier.The lease for the plaque is 10 years, but you can buy another one after this time.You can visit one of our authorised Memorial Masons who can help you to fill in yourapplication for the lease and prepare the plaque for you.For a list of Masons licensed to work in the city cemeteries see the our web page ortelephone the Bereavement Care Service office for an up to date list.Please note that although you can buy the plaque from the Memorial Mason you canonly lease the space where it is placed on the train.Please contact us to place an order or for more information

31Tree and markerOne of the most lasting ways to remember a loved one is by sponsoring a tree intheir memory.You can sponsor a tree at Carmountside Cemetery and Crematorium and in all of thecity's cemeteries except Trentham Cemetery.There are a wide range of beautiful mature trees available to sponsor.In some cases we could consider planting a new tree. This depends on where youwant it, and the type of tree. Please contact us for specialist advice on this.A tree has a marker bearing a suitable message. The lease for a marker is 10 or 20years, but you can buy another one after this time.You can make an appointment with us if you would like to witness the marker beingplaced.We have responsibility for maintaining the tree throughout its life. Pruning or otherwork, will be carried out as needed. We will replace the tree if it dies or becomesdangerous during the lease period.Please contact us to place an order or for more information

32Vase tabletVase tablets are available in the Garden of <strong>Remembrance</strong> at CarmountsideCemetery and Crematorium, and the other city cemeteries (apart from Tunstall andTrentham).The vase is made of granite and is available in four different colours:• black;• grey pearl;• blue pearl; and• pink pearl.They have an inscription (message) on the front.The vase can be put as near as possible to where cremation remains have beenscattered, but not actually on the strewing area.Locations of vases must be agreed with a member of Bereavement Care Servicestaff prior to ordering form a Memorial Mason. We will arrange a meeting to discussthis prior to application.Vase tablets can be bought from an authorised memorial mason licensed to sellthem. Bereavement Care Service can provide an list of approved memorial masons.The area where the vase is displayed is leased from us for 10 years, but you can buyanother lease after this time. If you choose not to we will remove the vase fromdisplay.You can get a list of authorised memorial masons licensed to work in the citycemeteries from the main reception at Carmountside Cemetery and Crematorium.Please contact us to place an order or for more information

33Wilford granite seat plaqueWilford memorial seats are an attractive and lasting feature in all of our citycemeteries except Trentham Cemetery.Each four foot polished granite seat will be shared at most by three families.The location of the seat is at our discretion. You will need to agree the position ofyour seat with us before it is ordered. We will arrange a meeting to discuss this priorto application.The plaque will display an inscription (message) of your choice in gold lettering.The lease for a plaque is 10 years, but you can renew it after this time.If you do not want to renew the lease we will remove the plaque from display andsuitably dispose of it.Please contact us to place an order or for more information

Out of station, turn right totraffic lights, turn left onto Leek Road A5009. Keep on road for approximately 2 milesthrough major crossroads and under disused railway bridge heading for Milton.Carmountside Cemetery and Crematorium is sign posted and is on the right handside.
