Let me begin by observing th<strong>at</strong> this was a ‘double-barrelled’ Guideline (as were most <strong>of</strong><br />
the others). Council expected us not only to impart the requisite tools (knowledge, skills,<br />
and <strong>at</strong>titudes) to the nascent <strong>doctors</strong> under our care, but to also ensure th<strong>at</strong> they would use<br />
these said tools “to deal with the health care problems <strong>of</strong> urban and rural communities, families and individuals in<br />
Transkei”<br />
The Eastern Cape Province (ECP) is basically a rural, and with an urban : rural popul<strong>at</strong>ion<br />
distribution <strong>of</strong> 35 : 65%, it is the third most rural province in the country after Limpopo<br />
(11 : 89) and KZN (39 : 61) 37 ;<br />

the doctor popul<strong>at</strong>ion r<strong>at</strong>ios. For example, <strong>of</strong> the 9 Provinces, the ECP has the third lowest<br />
number <strong>of</strong> <strong>doctors</strong> per 100 000 popul<strong>at</strong>ion (27) after Limpopo (18) and North West (23) 38,<br />
39 . And if th<strong>at</strong> is the case today, one can confidently say th<strong>at</strong> the r<strong>at</strong>io was eve lower in the<br />
Transkei <strong>of</strong> <strong>25</strong> <strong>years</strong> ago under apartheid and homeland rule.<br />
