No medication will be administered without the <strong>Florence</strong> <strong>Township</strong> <strong>School</strong>s ARequest for<br />

Administration@ form being filled out and signed by the doctor and the parental<br />

authorization filled out by the parent or guardian (signed and dated).<br />

All medications must be in pharmacy- labeled containers. The medication should be<br />

brought to the school in the original container, appropriately labeled by the pharmacy or<br />

physician.<br />

EMERGENCY/HEALTH CARD INFORMATION: Accurate emergency information<br />

regarding each student must be provided to assure proper care and safety. This<br />

information is requested on a card which is filed in our school=s Health Office. <strong>Parent</strong>s<br />

should complete and sign both sides of the emergency card. Please contact the <strong>School</strong><br />

Nurse=s office if, at any time during the school year, the Emergency/Health Card<br />

information needs to be updated or changed.<br />

<strong>Parent</strong>s of students with anaphylaxis are responsible for attending any school sponsored<br />

activity or trip. If they cannot attend, they must provide a delegate for epinephrine<br />

administration. If a delegate is unavailable, the parent must notify the school nurse at<br />

least two weeks prior to the event.<br />

END-OF-DAY ROUTINE<br />

DISMISSAL<br />

Prior to leaving school, students should follow a routine of checking their Agenda Books to<br />

ensure that they have books and materials they need from their lockers.<br />

EARLY DISMISSAL PROCEDURES<br />

Early dismissals are sometimes necessary for a doctor, dental or daytime appointment. The<br />

process used by our office is as follows:<br />

On the morning of the early dismissal, the student should report to the Main Office<br />

before school and turn in his/her parent or guardian=s written notification to the office<br />

staff.<br />

The student will receive an AEarly Release Form@ that should then be presented to the<br />

teacher at the time that the student has to leave that day. (If the student=s early dismissal<br />

time will occur in the middle of a class period, the student should notify the classroom<br />

teacher prior to the beginning of that class period.)<br />

In the interest of our students= safety and school security, in the event of an early<br />

dismissal, parents/guardians must come into the Main Office in person to Asign out@ and<br />

accompany their children. Upon returning, we ask that the parent also returns to the<br />

office to sign the student back in.<br />

PLEASE NOTE: A student will not be released to an adult unless that adult is<br />

designated as an additional contact on the <strong>Student</strong> Information Card.<br />

The early dismissal will not be documented as Aexcused@ until an accompanying note from the<br />

doctor, dentist or legal agency is brought to the office, by either student or parent, following the<br />

appointment.<br />
