OPTN Member Evaluation PlanUpdated January 1, 2015Policy 14.3.A.i: Living Kidney Donor Informed Consent for Evaluation of PotentialLiving DonorsEffective Date: 2/1/2014At Living Donor recovery hospitals, site surveyors will:Review a sample of living donor medical records, and any material incorporated into the medical record byreference, for documentation that:⦁ The potential donor was offered an opportunity to discontinue the donor consent or evaluation process in away that is protected and confidential⦁ An ILDA was available to assist the potential donor during the processReview a sample of living donor medical records, and any material incorporated into the medical record byreference, for documentation that the recovery hospital provided information or disclosure to the donor stating that:⦁ The hospital may refuse the potential donor⦁ The following are inherent risks associated with evaluation for living donation:⦁ Allergic reactions to contrast⦁ Discovery of reportable infections⦁ Discovery of serious medical conditions⦁ Discovery of adverse genetic finding⦁ Discovery of abnormalities that may require additional testing at the donor's expense or create theneed for unexpected decisions by the transplant teamReview a sample of living donor medical records, and any material incorporated into the medical record byreference, for documentation that the recovery hospital provided information or disclosure to the donor addressingthe risk of the following:26⦁ Death⦁ Scars, pain, fatigue, and other consequences typical of any surgical procedure⦁ Decreased kidney function⦁ Abdominal or bowel symptoms⦁ Kidney failure and the need for dialysis or kidney transplant for the donor⦁ Impact of obesity, hypertension, or other donor-specific medical condition on the morbidity and mortalityof the potential donor⦁ Problems with body image⦁ Post-surgery depression or anxiety⦁ Feelings of emotional distress or grief if the transplant recipient experiences any recurrent disease or in theevent of the recipient's death⦁ Impact of donation on the donor's lifestyle⦁ Personal expenses of travel, housing, child care costs, and lost wages related to donation might not bereimbursed⦁ Need for life-long follow-up at the donor's expense⦁ Loss of employment or income⦁ Negative impact on the ability to obtain future employment⦁ Negative impact on the ability to obtain, maintain, or afford health, disability, and life insurance⦁ Future health problems experienced by living donors following donation may not be covered by therecipient's insurance⦁ Risks may be temporary or permanent⦁ Risks may include those listed in this policy, but are not limited to those listed
