<strong>OPTN</strong> Member <strong>Evaluation</strong> <strong>Plan</strong>Updated January 1, 2015Policy 2.6.A: Deceased Donor Blood Type DeterminationEffective Date: 2/1/2014At OPOs, site surveyors will:Review a sample of deceased donor records for the following documentation:⦁ The results of two blood typing tests⦁ If the two tests were completed on blood drawn at the same date <strong>and</strong> time, then documentation showingthat the tests were run by two different laboratoriesOPOs will provide:The requested sample of deceased donor medical recordsPolicy 2.6.B: Deceased Donor Blood Subtype DeterminationEffective Date: 2/1/2014At OPOs, site surveyors will:Review a sample of deceased donor records for the following documentation:⦁ When the donor's blood type was reported in UNet℠ as non-A1, that there are two results from two pretransfusiontypings that show the donor as non-A1.⦁ When the donor's blood type was reported in UNet℠ as non-A1B, that there are two results from two pre-transfusion typings that show the donor as non-A1B.⦁ If the two tests were completed on blood drawn at the same date <strong>and</strong> time, then documentation showing that the tests were run by two different laboratories⦁ If there are discordant subtyping results, subtype is not reportedOPOs will provide:The requested sample of deceased donor medical recordsAdditional Guidance:A hospital's electronic medical record (EMR) can be a source document for lab results when the performing lab'sresults are directly uploaded into the EMR.3
