The completed application must:⦁ Demonstrate that the proposed surgeon or physician meets the primary surgeon or physician requirementsfor that organ⦁ Be received by the OPTN contractor at least 30 days before the effective date of the change in key personnel(due date will be provided by staff)If the program received less than 60 days advance notice of a primary physician or surgeon's departure orneed for temporary leave, the application must be submitted to the OPTN contractor within 30 days after theprogram notifies the OPTN contractor of the pending change (due date will be provided by staff)Voluntarily inactivate or withdraw its designated transplant program status, if:⦁ The transplant program's primary surgeon or physician ends active involvement with the program on apermanent or temporary basis, and⦁ The program is unable to:⦁ Submit a completed key personnel application by the due date⦁ Demonstrate in the application that the proposed replacement meets the primary surgeon or physicianrequirementsNotify the OPTN contractor in writing of a change in key personnel within seven business days of transplantprogram knowledge of the departure or extended absence of the program's primary surgeon or physician(including primary living donor surgeons).Where to send notification:⦁ By Mail: UNOS Member Quality700 N 4th StreetRichmond, VA 23219⦁ By Facsimile: (804) 782-4896⦁ By E-mail: to your regional Application Related contact(http://transplantpro.org/wp-content/uploads/Regional_Map_Staff.pdf)54
OPTN Member Evaluation PlanUpdated January 1, 2015Bylaws Appendix D.6.D: Reinstatement of Previously Designated PrimarySurgeon or Primary PhysicianEffective Date: 9/1/2012How the OPTN contractor will evaluate member compliance with this bylaw:The OPTN contractor will review written reinstatement requests.Transplant hospitals must:Submit a written reinstatement request if the program wishes to reinstate a previously-designated primarysurgeon or primary physician who left the hospital and returned.
