<strong>OPTN</strong> Member <strong>Evaluation</strong> <strong>Plan</strong>Updated January 1, 2015Bylaws Appendix K.5: Transition <strong>Plan</strong> during Long-term Inactivity, Termination, orWithdrawalEffective Date: 9/1/2012How the <strong>OPTN</strong> contractor will evaluate member compliance with this bylaw:The <strong>OPTN</strong> contractor will investigate any allegations of noncomplianceThe <strong>OPTN</strong> contractor will review materials submitted by members:⦁ Transition plans⦁ Routine reportsTransplant hospitals must:Submit a transition plan to the <strong>OPTN</strong> contractor within seven days of the effective date (may be submittedseparately from the initial notice). It must include:⦁ A list of c<strong>and</strong>idates on the transplant hospital's waiting list, with the following information on eachc<strong>and</strong>idate:⦁ If the c<strong>and</strong>idate chose to transfer to another hospital, the program to which the c<strong>and</strong>idate is transferring⦁ If the c<strong>and</strong>idate chose not to transfer to another hospital:⦁ The reason⦁ Whether the c<strong>and</strong>idate was informed of the implications of removal from the waiting list⦁ A list of the most urgent c<strong>and</strong>idates, including:⦁ Individualized plans for transfer⦁ Potential alternative transplant programs⦁ Timeline for transferring those c<strong>and</strong>idates according to priorities <strong>and</strong> deadlines listed in Bylaw K.5(6)Submit routine reports to the <strong>OPTN</strong> contractor until the program has completely cleared its waiting list of both active <strong>and</strong> inactive c<strong>and</strong>idates. In general, these reports are due on the 1st <strong>and</strong> 15th of each month.Immediately stop organ transplantation.Help potential c<strong>and</strong>idates <strong>and</strong> c<strong>and</strong>idates transfer to other programs.Transfer c<strong>and</strong>idates to another hospital when either:⦁ Requested by the c<strong>and</strong>idate⦁ The c<strong>and</strong>idate is active <strong>and</strong> currently hospitalized at the transplant program. Then the transplant programmust:⦁ Initiate the transfer within 14 days after inactivation, withdrawal or termination, unless any of thefollowing:⦁ Transfer would be unsafe⦁ Discharge is anticipated within the 14 day time period⦁ Circumstances outside the transplant hospital prevent transfer within 14 days⦁ Document all efforts to transfer these c<strong>and</strong>idates <strong>and</strong> submit that documentation to UNOS if a programcannot meet these deadlinesWhere to send notification:⦁ By Mail: UNOS Member Quality700 N 4th StreetRichmond, VA 2321965
