Bylaws Appendix B.4: OPO Personnel<strong>OPTN</strong> Member <strong>Evaluation</strong> <strong>Plan</strong>Updated January 1, 2015Effective Date: 9/1/2012How the <strong>OPTN</strong> contractor will evaluate member compliance with this bylaw:The <strong>OPTN</strong> contractor will investigate <strong>and</strong> refer a member to the MPSC for review when:⦁ A member fails to inform the <strong>OPTN</strong> contractor of a change in key personnel within 30 days of departure.⦁ A member fails to submit the replacement's name <strong>and</strong> curriculum vitae no less than 30 days before thechange will take effect.OPOs must:Submit written notice immediately (<strong>and</strong> within 30 days) after learning that the OPO administrative directoror medical director plans to leave or otherwise change positions <strong>and</strong> no longer serve in one of these roles.Written notice of a change in key personnel must include:⦁ Name of new director⦁ Status of appointment (interim or permanent)⦁ Effective date of the change⦁ A current curriculum vitaeNotify the <strong>OPTN</strong> contractor if it has not filled a vacant administrative or medical director positionpermanently within six months.
