important considerations for all indications<strong>Nobel</strong>Guide is intended for single, partial and fullyedentulous jaws where the patient:• meets general health requirements for undergoingoral surgery.• is fully healed after any grafting procedures.• has a sufficient amount of jaw bone.• has sufficient mouth opening capability toaccommodate the surgical tooling.Checklist before Surgery• Make sure that you have the correct instruments,components. important considerations for all indicationsImportant information about Surgical Templateproduced by <strong>Nobel</strong> <strong>Biocare</strong>• The Surgical Template is made of a material that issensitive to moisture and UV light. Store the SurgicalTemplate together with a moisture absorbent in theUV protective plastic bag in which it was delivered.• Always store the model in a dry and dark location.• Never expose the Surgical Template to directsunlight.• Never remove the moisture absorbent.• Disinfect in chlorhexidine for a maximum of 20minutes. Dry quickly, but without using heat.Caution! The Surgical Template may deform if exposed toliquids for more than 30 minutes.Common Drilling ConsiderationsCaution! Before all drilling, make sure the drills move freely andeasily through the template sleeves and/or drill guides, downto the soft tissue.Start drilling with the drill in the template sleeve and/ordrill guide. Avoid lateral pressure on drills when drilling.Lateral pressure may damage the drill.The drills are made from surgical stainless steel and arecoated with an amorphous diamond coating, which gives themtheir black color.• Use an in-and-out motion and drill in bone for 1–2 seconds.• For all drills (Start Drill/Counter Drills and Twist Drills) set thedrilling unit on high speed and 2000 rpm.High speed2000 rpm• For screw taps and implant placement the drilling unit shouldbe set on low speed and the torque between 20–45 Ncm.Low speedMax 45 NcmNote! All Drills and Screw Taps are disposable and should be usedfor one surgery only. Do not re-sterilize disposable drills.11


important considerations for all indicationsImplant MountingOpen the outer packaging (which is non-sterile) of the implantand empty the inner sterile container with the implant ontothe sterile field.• Remove the plastic cylinder.• Place the titanium cylinder in the implantsleeve holder.• Connect the Guided Implant Mount <strong>Nobel</strong>Replace RP to the implant using a Screwdriver Unigrip and thesurgical adapter from Manual Torque Wrench.• Pick up the mounted implant with the <strong>Nobel</strong>Replace Connection to handpiece.Drill Stop• Mount a Drill Stop on Guided Twist Drills for a safe andaccurate drilling procedure.• Insert the Guided Twist Drill with drill stop in a mountinghole with a depth corresponding to the depth of theosteotomy.• Use the larger holes for drills ∅ 3.4 and above.• Tighten using a Screwdriver Unigrip .• The Drill Stop is now mounted at a height where it servesas a hard stop when drilling to the desired depth througha Drill Guide mounted in a Surgical Template.12


important considerations for all indicationsInspection WindowsIn single and partial cases, Inspection Windows are grindedthrough the top of the Surgical Template allowing inspectionof the underlying dentition, thus confirming the proper seatingof the Surgical Template.• The Inspection Windows should be monitored throughoutthe surgery in order to verify correct seating.Retention PrinciplesIn full and partial cases, the correct position of the SurgicalTemplate is ensured by Anchor Pins. Retention can be furtherenhanced by holding the Surgical Template onto the existingdentition using hand pressure.• In single cases, no Anchor Pins are used. Retention isobtained by pressing the Surgical Template onto existingdenture.• A suture can also be tied around the Surgical Template andthrough an inter-dental opening, thus tying the SurgicalTemplate to the dentition.13

Prosthetic Procedure – TemporizationWhen carrying out a Guided Surgery prosthetic procedure, awide range of <strong>Nobel</strong> <strong>Biocare</strong> abutments can be used, such as:• Immediate Temporary Abutment• Procera ® Abutment• Snappy Abutment • Esthetic Abutment For the purposes of this indication, we show Procera ®Abutment.ONEAbutment Connection• Connect the Procera ® Abutment using the ScrewdriverUnigrip and tighten manually.• Verify the correct abutment connection with radiographicimaging.Tightening• For the final tightening, use the Manual Torque WrenchProsthetic and Screwdriver Unigrip Machine. The abutmentscrews should be tightened to 35 Ncm.• If required, occlusal adjustments have to be performed.Cementation of Temporary• Cement the temporary crown• Follow established prosthetic procedures to make the finalrestoration after sufficient healing period.17

The dots on the Implant Mount indicate the position of theinternal connection tri-channel lobes.• Release the Implant Mount with a Screwdriver Unigrip .Note! procedure: several teeth missingAnchoring the Surgical Template• The Surgical Template can now be anchored to theimplant using a Guided Template Abutment<strong>Nobel</strong>Replace RP.• Use a Screwdriver Unigrip to tighten manually.SEVERALInstallation of the Remaining Implants• After the Surgical Template has been tightly attached withthe Anchor Pin and to the first implant, proceed with thepreparation of the remaining implant sites.• Install the remaining implants according to the proceduredescribed previously.• Leave Implant Mount on to keep soft tissue out of the way.Note! No template abutments need to be placed on theseimplants.Removal of the Surgical Template• When all the implants have been properly placed, removethe Implant Mounts and Template Abutments.• Use the Guided Soft Tissue Punch RP directly in thetemplate sleeve to completely remove all soft tissue.• Remove Anchor Pin and Surgical Template.22


Prosthetic Procedure Teeth-in-an-Hour Mounting Guided Abutment <strong>Nobel</strong>Replace RP in Procera ®Implant Bridge.• Before the sterile abutments are placed, make sure thebridge is thoroughly disinfected.• Moisten the abutments by quickly dipping them in sterile,saline water.• The Guided Abutments <strong>Nobel</strong>Replace RP are placed inthe Procera ® Implant Bridge.Note! Following placement of the abutments, do not turn thebridge upside down. This to avoid the abutments falling out!Delivery of Procera ® Implant Bridge• When the abutments have been placed in the bridge, thebridge is positioned onto the implants and screw-retainedusing the Screwdriver Unigrip .• After the bridge has been inserted, use the ScrewdriverUnigrip to tighten to first resistance and ask the patientto bite gently to balance the bite relationship.SEVERALTightening• For the final tightening, use the manual torque wrenchprosthetic and Screwdriver Unigrip machine. The abutmentscrews should be tightened to 35 Ncm.• Verify the correct abutment connection with radiographicimaging.• If required, occlusal adjustments have to be performed.Temporarily seal the screw access holes.• Control occlusion and function after 1 month.Check screw tightening to 35 Ncm.Final sealing of screw access holes.23

The abutmentscrews should be tightened to 35 Ncm.• If required, occlusal adjustments have to be performed.Cementation of Temporary• Cement the temporary restoration.24

Do not re-sterilize disposable drills.Instruments• Devices must be cleaned and sterilized before intra oraluse in accordance with established procedures in thehospital, clinic or office.Principal Cleaning and Sterilization Procedure• Clean and disinfect instruments and kits in a dishwasher.Alternatively: Disinfect, clean by hand and put in an ultrasoniccleaner.• Dry instruments and put them back in the kit. Place insterilization packets.• Sterilize instrument using a steam autoclave (follow manufacturer’sinstructions).Contra-Angle Cleaning• Contra-angle must be carefully cleaned immediately afteroperation.• Clean first in a washer or under running water.
