the circumstances, the line manager 3 will report concerns identified to theSocial Work Service.2.2 Duty to reportStaff have a duty to report suspicions or disclosures made about any adult atrisk of harm. While this may cause the individual staff member difficulties, afailure to report is a failure to carry out a duty of care. Staff must report anyconcerns of suspected or actual harm to their line manager.If the adult does not give consent to report the harm go to 2.72.3 Whistle blowing/raising concernsOrganisations should have policies and procedures in place to deal withemployee concerns about unprofessional, dangerous or illegal activitieswhich they become aware of through their work. An essential element of suchpolicies is the underpinning principle that staff who raise concerns reasonably,responsibly and in good faith will not be penalised or victimised in any way.For further information staff should refer to the relevant “Whistle blowing”policy for their own particular organisation.Person Responsible:Staff must report any concerns of suspected or actual harm to their linemanager. The line manager then must take appropriate action.Actions to be taken:Contact the <strong>Adult</strong> <strong>Protection</strong> Phone Line on Tel: 01383 602200Provide as much relevant detail to assist the Social Work Service namely:<strong>Adult</strong>’s details including: name, date of birth, ethnic origin, gender, religion,family circumstances, support networks, physical or mental health concerns,and communication difficulties.The nature of the concern or allegation and alleged perpetrator.Own job title and contact details.Record all actions taken in line with your agency’s recording requirements.Submit an <strong>Adult</strong> Cause for Concern Form to the Social Work Contact Centre.(See Appendix 2)3 Exceptions to this would be where your immediate line manager is not available or wherethe line manager is implicated- refer to local guidance.18
