The definition of doctor, nurse andmidwife is as specified under their respectiveprofessionals Acts, i.e. Medical Act 1983 and Nurses &Midwives Order 2001.These are any records, in any format, which relate to anindividual’s physical or mental health which have beenmade by or on behalf of a health professional inconnection with the care of the individual.A member of an advocacy service which operatesindependently of other service providers. Advocacy ofteninvolves speaking up for the individual, helping them toexpress their views and assisting them to make their owndecisions and contributions.
