This is a significantdifference. The terms are not interchangeable. The ASPAct supercedes the No Secrets <strong>Guidance</strong>, and refers toall harmful conduct, including neglect and self-harm,which are not covered in the No Secrets <strong>Guidance</strong> or bythe definition of abuse. Abuse relates to deliberate harmcaused by another, while the term Harm is wider andincludes both deliberate and unintentional acts oromissions.A module within the SWIFT system (see below) whereSocial Work staff record all actions and decisions madeunder the adult protection procedures. This includesInter-agency Referral Discussions (IRDs) andinvestigations undertaken by the Social Work Service.A person aged 16 years or over, unable to safeguard hisor her own wellbeing or property, is affected by disability,mental disorder, illness or infirmity, and is more at risk ofbeing harmed than other adults who are not so affected.A multi-agency meeting that will usually include the adultat risk of harm and whoever they choose to supportthem. The case conference will develop an adultprotection plan.A plan agreed by all agencies and the adult detailing themeasures provided to protect the adult at risk from harm.71
