A copy of this form will be submitted to the Ministry for their records.FREEZE POLICIES:• A freeze period starts on the first day of a month and ends on the last day of the month. (i.e. April 1 to June 30.)• Freeze periods may only be applied when class is not in session. For example, if your term ends May 15, the first monthof your freeze will be June, not May.• The freeze request should ideally be submitted one month before the start date of the freeze period, or as soon aspossible.• While on a freeze, a student is not eligible for any scholarship benefits including allowance payments, health insurance,tuition and fees, and dependent allowances or benefits.• If traveling outside of the United States or Canada, the student must inform the Academic Manager of his/her return tothe United States or Canada by submitting the Returning Student form and immigration documents, an addition to anyother information required by CBIE.• If a student freezes his or her scholarship after exceeding the period for English as a Second Language (ESL) studiesand does not have academic admission, the student may continue ESL by paying his/her own tuition fees and otherexpenses. (Tuition, health insurance, and monthly living allowance will be placed on hold during this time.) The studentmust maintain communication with his/her academic manager and provide updates on ESL grades and academicapplications. Full academic admission is required to reinstate scholarship benefits.• While on a freeze, the student is responsible for arranging health insurance coverage for himself/herself and his/herdependents.PLEASE READ THE FOLLOWING DISCLAIMER:I acknowledged that by requesting a freeze of my scholarship, I am not eligible for scholarship benefits during the period listedabove. Scholarship benefits include monthly living allowance, health insurance, tuition and mandatory fees. I will notify myAcademic Manager at the end of my freeze period and provide him/her with the documents required to reinstate my scholarshipbenefits. I understand that I am responsible for arranging health insurance coverage for myself and my family members duringthe freeze period.
