PART 3, APPLICANT INFORMATION:Name:Enter your legal name as it is on your currentlicence. If it has not changed, check the firstbox to indicate as such. If you have a new legalname, check the second box that indicates thereis a change and enter the new legal name in thefield beneath this check box. You must include acopy of the legal documentation that shows thename change when you send in your renewalapplication package.Citizenship:Check off only one of the three boxes and followthrough by providing the documentation listedto prove citizenship or your legal entitlement towork in Canada.Physical Description:If there have been any changes to the physicaldescription listed on the back of the licence youare renewing, enter the new information in thissection. You have a choice where you wantthe licence to be mailed. If you ALSO wish toreceive an electronic copy to your e-mail address(which means you will have a ‘temporary’ proofof licence so you may start working right awayin compliance with the Act which states youmust have your licence on you at all times whileproviding security services. The original licencewill be sent to your mailing address, but it maytake a few days to arrive.)Photograph:You may provide a different photo of yourself aslong it meets the criteria as stated on the “Pre-Licensing Requirements - Photograph” section ofthis guide (page 10).
