11/22/2011Clinical Features Of Cirrhosis• Nonspecific clinicalmanifestations: anorexia, weightloss, weakness, etc.• Jaundice• Sign of Progressive hepaticfailure• Portal hypertension;– ascites– the formation ofportosystemic venousshunts– congestive splenomegaly– hepatic encephalopathyJaundice• Jaundice จะปรากฏเมื่อระดับของ serum bilirubin levels มากกวา2.0 to 2.5 mg/dLในโรคบางโรคที่รุนแรงอาจจะมากกวา 30 to 40mg/dLBilirubin and Bile Formation• Bilirubin - ไดจากการสลายของ heme– ปกติปริมาณของbilirubin ที่ผลิตขึ้นตอวัน = 0.2 to 0.3 gm ไดจากการแตกสลายของเม็ดเลือดแดงที่แก ใน มาม ตับ และไขกระดูก– Bilirubin สวนที่เหลือมาจาก the turnover of hepatic heme orhemoproteins (e.g., the P-450 cytochromes) และจากการสลายของเม็ดเลือดแดงที่ยังไมเจริญเต็มที่ในไขกระดูก– อัตราการสราง bilirubin จะเทากับอัตราการดึงกลับของตับ/hepatic uptake conjugation และ การขับออกทางน้ําดีPathogenesis of Jaundice• Excessive production of bilirubin: Hemolytic anemia, ineffectiveerythropoietic sysdrome etc.• Reduced hepatocyte uptake: Drug interference with membranecarrier systems• Impaired conjugation: UGT1A1- Physiologic neonatal jaudice/Breast-fedding jaundice, Gilgert syndrome, Crigler-Najjarsyndrome etc.• Decreased hepatocellular excretion: Dubin-Johnson syndrome,Rotor syndrome etc.• Impaired bile flow (both intrahepatic & extrahepatic)24
