Safe Work Procedures & <strong>Confined</strong> Space Entry Permiti) Have safe work procedures been established for allconfined space works?ii) Do the safe work procedures for confined space coverthe following key areas?• Evaluation of the need to enter or carry out workin the confined space;• A confined space entry permit;• The types of atmospheric testing required <strong>and</strong>the interpretation of results of the tests;• The safety <strong>and</strong> health precautions to be takenduring entry into the confined space <strong>and</strong> duringan emergency situation;• The provision <strong>and</strong> safe use of safety equipment<strong>and</strong> PPE; <strong>and</strong>• The means to prevent unauthorised entry intothe confined space, including the display ofwarning signs.iii) Are the following information provided on the confinedspace entry permit?• The location <strong>and</strong> identity of the confined space;• The purpose of entry into the confined space;• The results of the gas testing of the atmosphere ofthe confined space; <strong>and</strong>• The validity of the confined space entry permit.iv) Is a copy of the confined space entry permit <strong>and</strong> asketch of work area displayed prominently at theentrance to the confined space?v) Are identification badges of entrants displayed at theentrance to the confined space?vi) Are the entry permits kept for a minimum of 2 yearsfrom the date of approval?5. Atmospheric Testing <strong>and</strong> Monitoringi) Is the space tested by a CSSA for oxygen levels,flammable <strong>and</strong>/or toxic gases, vapours or fumes beforean entry permit is issued?ii) Are the test results within the acceptable criteria?iii) Are the results recorded on the entry permit?iv) Is the atmosphere in the space tested at periodicintervals subsequently?v) Are the periodic test results recorded?6. Ventilation Provisionsi) Is adequate ventilation provided <strong>and</strong> maintained at alltimes in the confined space during the validity of theconfined space entry permit?i) Is adequate ventilation provided <strong>and</strong> maintained at alltimes in the confined space during the validity of theconfined space entry permit?ii) Is the air supply for the ventilation system from a sourceor area which is free of contaminants?7. General <strong>Safety</strong> Provisionsi) Are safe means of access <strong>and</strong> egress provided for themovement of entrants to <strong>and</strong> within the confined space?ii) Are all pressurised confined spaces depressurised <strong>and</strong>rendered safe prior to opening?iii) Are all confined space openings barricaded or guardedproperly after opening?iv) Are all confined space openings covered effectively toprevent objects from falling through?v) Is there sufficient <strong>and</strong> suitable lighting provided forentry into <strong>and</strong> work in confined space?vi) Are all electrical installation <strong>and</strong> equipment usedof good construction, sound material <strong>and</strong> properlymaintained?vii) Are all moving parts <strong>and</strong> equipment inside the confinedspace locked out <strong>and</strong> tagged?viii) Are warning signs posted at the entrances of confinedspace?8. Emergency Preparedness <strong>and</strong> Rescuei) Has a written rescue plan been established?ii) Are sufficient supplies of rescue equipment provided/made readily available?iii) Are the rescue equipment properly maintained?iv) Are records of every test <strong>and</strong> examination of therescue equipment by a competent person available forinspection?v) Are regular drills conducted?9. Trainingi) Have the persons entering the confined space receivedadequate safety <strong>and</strong> health training pertaining to thehazards associated with entry/work in the confinedspace?ii) Have all appointed rescue personnel received adequatetraining in rescue operation, including first aid <strong>and</strong>proper usage of PPE <strong>and</strong> other rescue equipment?iii) Are training records maintained <strong>and</strong> made available forinspection?78 ii) Is the air supply for the ventilation system from a source79or area which is free of contaminants?
