If a Client has a Treatment PlanThe objectives (“in progress) of the client will beavailable to address with each progress note.This is why it is important to personalizeobjective goals and not focus on programfocusedgoals-as in, “Client will submitrandom UAs”
Progress NotesA progress note is required for all clinical events suchas individual or group therapy sessions. In some cases,such as for residential services, therapeutic eventsoccurring over a number of days can be combined intoone progress note. In this case, the units would equalthe number of days covered by the progress note, andclient activities would be recorded in daily chart notesfor summarization into a weekly progress note.Reminder: When a treatment plan objective is beingreplaced with another because the client isn‟tachieving the desired results, or if it is resolved, thestatus of that objective should be changed so onlycurrent objectives appear in the NHIPPS ProgressNotes.
