There is a charge for thisservice.Important Note: SHS is not equipped tohandle life-threatening emergencies and doesnot provide complete 24-hour coverage. It alsodoes not provide specialty care in many areas.Therefore, any accidents or illnesses whichcannot be handled by SHS will be referred tolocal hospitals, facilities, or physicians. All costsincurred will be at <strong>the</strong> student’s expense. Besure that you have adequate health insurancecoverage. For information on universitysponsored health insurance, call <strong>the</strong> StudentInsurance <strong>Office</strong> at (805) 893-2592.• PAYMENT OPTIONSThere are two options available to undergraduatestudents:1. A student can pay as <strong>the</strong>y go. We acceptcash or checks (made out to U.C. Regents) or wewill bill <strong>the</strong>ir charges to <strong>the</strong>ir BA/RC account.2. A student can pre-pay for health carethrough a program called PATH (Prepaid AccessTo Health care). By signing up for PATH,students do not have to pay <strong>the</strong> visit orprocessing fees each time <strong>the</strong>y come to StudentHealth.• HEPATITIS BState law requires <strong>the</strong> Hepatitis B series becomplete by <strong>the</strong> time <strong>of</strong> enrollment if you are 18years <strong>of</strong> age or younger by <strong>the</strong> first day <strong>of</strong>classes. This information and form was sent toyou with your Student Intent to Register (SIR)packet and should be returned as soon aspossible so registration for classes is notimpacted. Forms and information can also befound at www.sa.ucsb.edu/studen<strong>the</strong>alth/.• MEDICAL REQUIREMENTSAll intercollegiate athletes are required to havea physical examination at <strong>the</strong> SHS prior toparticipation. International students andstudents born outside <strong>of</strong> <strong>the</strong> United States mustprovide TB clearance. This can be obtained atSHS during <strong>the</strong> first two weeks <strong>of</strong> <strong>the</strong> quarter. Ifyou have withdrawn for medical reasons and arereturning after an absence <strong>of</strong> less than one year,request that your personal physician send aletter to <strong>the</strong> Student Health Service indicatingthat your health is improved enough to continueyour studies. This letter should include anyrecommendations regarding limitations <strong>of</strong>physical activities, etc.Transportation and Parking Services• PARKING REGULATIONS & PERMITSUCSB parking permits are required on allvehicles parked on campus from 7:30 a.m. to10:00 p.m., Monday through Friday, andSaturdays and Sundays from 7:30 a.m. to 10:00p.m. (7days per week). No public parking isallowed between midnight to 5:00 a.m.,Saturdays and Sundays. Permits are not requiredon university administrative holidays. Pleasecheck <strong>the</strong> Transportation & Parking website atwww.tps.ucsb.edu for complete, updatedparking information.Parking at UCSB is not assigned; it is providedon a “first-come, first-served” basis. Faculty,staff, and students may park in “A”, “S”, or “C”lots with <strong>the</strong> appropriate permit.Short-term permits (such as 3 hour, daily,evening, and weekend) are sold at <strong>the</strong> campusparking <strong>of</strong>fice (across from Harder Stadium inLot 30), and from permit dispensers locatedthroughout campus parking lots.Long-term permits (such as annual andquarterly) are sold online via <strong>the</strong>permitstore.com at www.tps.ucsb.edu.Valid parking permits are required in all timezones from 7:30 a.m. to 10:00 p.m. Mondaythrough Sunday (7 days per week). This includes<strong>the</strong> time zones in front <strong>of</strong> on-campus residencehalls. Thisincludes:• Service Areas marked 24-Hours• Restricted Service Areas• Accessible (Disabled) Spaces (DMV issuedplacard required)• West Campus / Devereux Loop• Lot 2 (B-1 residential lot)• Lot 3 (northwest quadrant <strong>of</strong> lot, nearDavidson Library)• Lot 6 (“A” and “A” & “S” in sou<strong>the</strong>rn part <strong>of</strong>lot near MSI)• Lot 12 (“A” spaces near Phelps Hall)A valid parking permit is one that: a) has notexpired, b) is displayed properly (in <strong>the</strong> lowerleft corner on <strong>the</strong> dashboard/inside <strong>of</strong> <strong>the</strong> frontwindshield), and c) is used to park in a spacedesignated for that specific type <strong>of</strong> permit.• PERMIT ELIGIBILITYBecause parking on campus is extremelylimited, students living within two miles <strong>of</strong> UCSB(including Isla Vista and university owned <strong>of</strong>fcampushousing) are not eligible to purchase aparking permit. Students living outside <strong>the</strong> 2-mile limit may purchase a “C” permit and maypark in lots designated “C”. A limited number<strong>of</strong> parking permits are sold to on-campus,residential students.Residential parking permits and lot assignmentsare awarded by lottery, with no guaranteethat all requests can be accommodated.23• RESIDENTIAL VERIFICATION REQUIREMENTVerification <strong>of</strong> local residential address, such asa current housing lease agreement, is requiredin order to purchase a residential studentparking permit. At <strong>the</strong> time <strong>of</strong> sale, <strong>the</strong> studentwill be asked to provide pro<strong>of</strong> <strong>of</strong> local livingaddress, such as a current housing leaseagreement, utility bill, or BARC statement.• PARKING METERSParking permits are not valid in metered spaces(even with a parking permit, coin payment is stillrequired). Depending on <strong>the</strong> lot, <strong>the</strong> maximumtime varies from 20 minutes to 4 hours. Metersonly accept quarters. One quarter provides 10minutes <strong>of</strong> parking in all meters.• CAMPUS LIABILITYIndividuals park on campus at <strong>the</strong>ir own risk.Please lock your vehicle. The <strong>University</strong> <strong>of</strong><strong>California</strong> shall not be liable for any risk or loss<strong>of</strong>, or damage to, property <strong>of</strong> individuals,including vehicles or <strong>the</strong> contents <strong>the</strong>rein, whichmay result from <strong>the</strong> use <strong>of</strong> campus parkingservices or facilities.
