A complete list is available on<strong>the</strong>ir website at www.nslc.org.Because we send <strong>the</strong> Clearinghouse this data,in most cases, you only need to send yourlender a verification, if <strong>the</strong>y request clarificationon a “paper deferment request”.Federal Title IV Loan DefermentsSpecific Federal Title IV loan deferments areexempt from verification charges. To qualify,your deferment request should have <strong>the</strong>following wording in <strong>the</strong> title:• Federal Perkins (NDSL) Student Loan• Direct Loans for <strong>the</strong> Department <strong>of</strong> Education• Federal Family Education Loan Program(FFELP) for Stafford Bank Loans administeredby Sallie Mae, Citibank, Wells Fargo, orano<strong>the</strong>r lender.There is no charge for <strong>the</strong>se specific types <strong>of</strong>Title IV verifications. Rush fees and FederalExpress fees will still apply and need to bearranged, in advance, with <strong>the</strong> verifications staff.4. GraduationDegree evaluationsUndergraduates: It is your responsibility toreview your academic record regularly todetermine that you are making satisfactoryprogress towards graduation.Students in <strong>the</strong> College <strong>of</strong> Letters and Sciencewith 84 or more units completed may request aJunior Progress Evaluation in <strong>the</strong> college <strong>of</strong>fice.College <strong>of</strong> Letters and Science students with135 or more units completed may complete arequest for a Senior Progress Check from <strong>the</strong><strong>Office</strong> <strong>of</strong> <strong>the</strong> <strong>Registrar</strong>. Only one such requestwill be honored by each <strong>of</strong>fice. Progress checkswill not be performed if <strong>the</strong> student has alreadydeclared candidacy for graduation.Students in ei<strong>the</strong>r <strong>the</strong> College <strong>of</strong> Engineeringor <strong>the</strong> College <strong>of</strong> Creative Studies withquestions concerning academic requirementsshould contact <strong>the</strong>ir college <strong>of</strong>fice or majordepartment.The <strong>Office</strong> <strong>of</strong> <strong>the</strong> <strong>Registrar</strong> prepares an <strong>of</strong>ficialgraduation evaluation. It is designed as a finalclearance and will not be completed until <strong>the</strong>end, or after <strong>the</strong> close, <strong>of</strong> <strong>the</strong> quarter in whichyou declare your candidacy for graduation. It isnot intended to advise you <strong>of</strong> requirements yetto be fulfilled. Therefore, you are encouraged toreview your own records as carefully as possibleprior to <strong>the</strong> beginning <strong>of</strong> your final quarter andrequest <strong>the</strong> progress checks mentioned above.All work used toward completion <strong>of</strong> degreerequirements must be completed in a termending on or before <strong>the</strong> last day <strong>of</strong> <strong>the</strong> UCSBquarter in which you plan to graduate.Petitions for exceptions to and substitutionsfor degree requirements are available from yourcollege <strong>of</strong>fice.Declaration <strong>of</strong> CandidacyAll undergraduate candidates for a degree mustdeclare <strong>the</strong>ir candidacy for graduation by usingGOLD no later than <strong>the</strong> second week <strong>of</strong> classesin <strong>the</strong> quarter in which <strong>the</strong> degree is anticipated.(See “Deadlines” for specific dates.)Please note: declaring candidacy for graduationis a separate process from participating incommencement ceremonies. (See “Commencement”below.)Students who dis<strong>cover</strong> that <strong>the</strong>y are not eligibleto graduate must withdraw <strong>the</strong>ir candidacy byusing GOLD so that registration material for <strong>the</strong>following quarter will be prepared. Students mustdeclare candidacy for <strong>the</strong> new date <strong>of</strong> graduation,whe<strong>the</strong>r withdrawal from candidacy is madeby <strong>the</strong> student or as a result <strong>of</strong> action by <strong>the</strong><strong>Office</strong> <strong>of</strong> <strong>the</strong> <strong>Registrar</strong> due to deficiencies inmeeting graduation requirements.CommencementCommencement is held in June for studentswho received degrees in <strong>the</strong> precedingDecember and March, and for those studentswho are candidates for degrees in June orsummer. Please note: participating in commencementceremonies is a separate processfrom declaring candidacy for graduation. (See“Declaration <strong>of</strong> Candidacy” above.) Seniors whowish to participate in commencement mustmake a reservation on GOLD.In order for your name to appear in <strong>the</strong>commencement book, you must: a) declare yourcandidacy in <strong>the</strong> manner outlined above, and b)have completed or plan to complete all degreerequirements. Your name will not appear in <strong>the</strong>commencement book if you have requested towithhold disclosure <strong>of</strong> public information fromyour record.DiplomasDiplomas usually arrive several monthssubsequent to graduation. If you need <strong>of</strong>ficialverification that you did graduate after you havecompleted your final quarter, you may order an<strong>of</strong>ficial transcript from <strong>the</strong> <strong>Office</strong> <strong>of</strong> <strong>the</strong><strong>Registrar</strong>. Indicate <strong>the</strong> date and degree earnedon <strong>the</strong> request. (See “Transcripts, Official.”)If you complete your degree requirements butwill not graduate until <strong>the</strong> next <strong>of</strong>ficial graduationdate, and need immediate pro<strong>of</strong> that youhave finished your undergraduate education,you may request a letter <strong>of</strong> verification from <strong>the</strong><strong>Office</strong> <strong>of</strong> <strong>the</strong> <strong>Registrar</strong>. (See “Verification <strong>of</strong>Student Status.”) This letter will be issued only if1) your records are complete in <strong>the</strong> <strong>Office</strong> <strong>of</strong> <strong>the</strong><strong>Registrar</strong>, and 2) <strong>the</strong> degree evaluation has beenperformed and you have fulfilled all <strong>of</strong> yourgraduation requirements.

The centerprovides a high-quality child care program forchildren 3 months to 7 years <strong>of</strong> age in full- andhalf-day classes. Tuition varies depending on <strong>the</strong>age <strong>of</strong> <strong>the</strong> child and <strong>the</strong> number <strong>of</strong> days andhours <strong>of</strong> attendance. The center is open dailyfrom 7:30 a.m. to 5:30 p.m., Monday throughFriday. All students pay a reduced tuition rate.Low-income student parents may also beeligible for a State Department <strong>of</strong> EducationGrant for tuition subsidy. In addition, <strong>the</strong> Centerparticipates in <strong>the</strong> Federal Food Program, whichprovides nutritious meals to children whosefamilies are income eligible. We are located across from <strong>the</strong>Event Center between <strong>the</strong> residence halls andI.V. making it easy to access health care oncampus. We have our own parking lot withplenty <strong>of</strong> student parking (C sticker required) aswell as metered parking available.We <strong>of</strong>fer full service primary care as well aslimited specialty care. The following are some <strong>of</strong>our services available: urgent care, same dayappointments, women’s health (including annualexams), internal medicine, psychiatry, rheumatology,orthopedics, pharmacy, laboratory, x-ray,physical <strong>the</strong>rapy, allergy shots, immunizations,travel medicine, wart removal, HIV testing,minor surgery, accutane <strong>the</strong>rapy, health advising(alcohol & o<strong>the</strong>r drugs, nutrition, stressmanagement, eating disorders, sexual health),and physicals.Eye and/or Dental care are provided on a feefor-servicebasis. If you needemergency transportation, call <strong>the</strong> CampusRescue Squad at 9-911. There is a charge for thisservice.Important Note: SHS is not equipped tohandle life-threatening emergencies and doesnot provide complete 24-hour <strong>cover</strong>age. It alsodoes not provide specialty care in many areas.Therefore, any accidents or illnesses whichcannot be handled by SHS will be referred tolocal hospitals, facilities, or physicians. All costsincurred will be at <strong>the</strong> student’s expense. Besure that you have adequate health insurance<strong>cover</strong>age. A student can pay as <strong>the</strong>y go. We acceptcash or checks (made out to U.C. Regents) or wewill bill <strong>the</strong>ir charges to <strong>the</strong>ir BA/RC account.Visitsare $36 per visit, lab processing fees are$15/day and x-ray processing fees are $20/day.There are additional charges for o<strong>the</strong>r services.2. A student can pre-pay for health carethrough a program called PATH (Prepaid AccessTo Health care). For $200/year (includingsummer) or $75/quarter a student would nothave to pay <strong>the</strong> visit or processing fees eachtime <strong>the</strong>y come to Student Health. These feesare included in PATH.Fees are subject to change.
