Since the opening of the Jewish Home at Rockleigh, our
goal in the Nursing Department has remained the same.
The promise to provide the highest quality of care in a
compassionate manner is truly important and at the core
of our mission.
Quality nursing care is always our concern. This year,
Nursing is focusing on various quality indicators including
the prevalence of falls, nosocomial pressure ulcers, urinary
tract infections, and decline in resident’s function. Quality
assurance is our means of ensuring that we meet the
health needs in our setting and develop interventions in
our resident population based on evidence review.
Navigating the way to effective care transitions is also
a focus of the Nursing team this year. This area not only
impacts the long term care setting but also hospitals,
assisted living facilities and residents’ homes. The goal of
this program is to create strategies and interventions to
decrease frequent readmission to hospitals, decrease
unplanned and avoidable transfers and provide resident
centered care planning. Unplanned transitions greatly
impact our residents, family and caregivers both
physically and emotionally, and are very costly.
Electronic medical records are an integral element of
good care transition. The system enables the facility
to have a standardization and accurate communication
between facility settings, doctors, practitioners and
nurses. This will promote effective handovers, medication
reconciliation and safe medication practices. This year
we are excited about working toward introducing
computerized medical records in our facility.
With changes in our medical trends and increased
resident acuities, it is the responsibility of the Nursing
Department to assure optimum care and to continually
improve our services.

Each and Every Day
As we take the opportunity to reflect upon the past year
at Jewish Home Assisted Living, we have seen our
community grow in many ways. We have welcomed
many new residents, experienced the loss of some, and
seen a large number of our residents age in place. Our
committed staff of nurses and care associates, many of
whom celebrated their five year anniversary at the Jewish
Home Assisted Living, continue to strive to deliver the
highest quality of care each and every day.
We are truly blessed to
have had the opportunity
to love and care for
so many wonderful
residents and families
over the past five years.
We continue to be
privileged with the
ability to care for eight
of our original residents of JHAL which reflects our true
commitment to individuality, dignity, respect, quality
of care and of course, trust. Trust has been the key to
the success of the Wellness Department at JHAL.
Families truly trust that their loved ones are in the hands
of dedicated, loving nurses and care givers. Their dedication
and compassion to the residents is made obvious
each and every day!
Not one department is responsible for making JHAL
the outstanding community it is, but the whole interdisciplinary
team, from our executive director, marketing,
housekeeping, maintenance, nursing, social work and
dietary. Each of us, as a member of the team, brings a
very important piece to the JHAL family and our continued
commitment to the best quality of care for each of
our residents. This team of highly dedicated and caring
professionals strives each and every day to make JHAL
the absolute best community.
