Most hospitals manage the files around physician privileging with paper files. Many responses indicated to refer to question 8 from section one, since the process<br />

is the same.<br />

9. What, if any information related to physician performance, do you report to your HA?<br />

In some cases no information related to physician performance is reported to the respective HA. When information is reported, it includes:<br />

them.<br />

Greater than half of the responses indicate there are no instances where information related to physician performance would be reported to the Ministry.<br />

However, some sites stated they would report:<br />
