On May 12, 1998, Licensee entered a plea <strong>of</strong> guilty to<br />
the <strong>of</strong>fense <strong>of</strong> DWI - 1st in the Circuit Court <strong>of</strong> Marion<br />
County, <strong>Missouri</strong>. On October 26, 2016, Licensee pled<br />
guilty to the class C felony <strong>of</strong> Possession <strong>of</strong> Controlled<br />
Substance - Methamphetamine in the Circuit Court <strong>of</strong><br />
Ralls County, <strong>Missouri</strong>. Licensee reported her drugs <strong>of</strong><br />
choice were methamphetamine, marijuana and alcohol,<br />
with a lengthy history <strong>of</strong> abuse.<br />

on February 8, <strong>2017</strong>, Respondent failed to call in or<br />
check in with NTS on thirty-four (34) days that were<br />
not excused. Further, on July 15, 2016; November 14,<br />
2016; and December 22, 2016, Respondent called NTS<br />
and was advised that she had been selected to provide<br />
a urine sample for screening. Respondent failed to<br />
report to a collection site to provide the requested<br />
sample. In addition, on <strong>August</strong> 23, 2016, and January<br />
30, <strong>2017</strong>, Respondent failed to call or check-in with<br />
NTS; however, both were days that Respondent had<br />
been selected to submit a sample for testing. Therefore,<br />
Respondent failed to report to a collection site to<br />
provide a sample for testing on <strong>August</strong> 23, 2016, and<br />
January 30, <strong>2017</strong>. The <strong>Board</strong> did not receive an<br />
employer evaluation or statement <strong>of</strong> unemployment by<br />
the documentation due date <strong>of</strong> January 11, <strong>2017</strong>.<br />

on October 19, 2016, the <strong>Board</strong> had not received pro<strong>of</strong><br />
<strong>of</strong> any completed hours.<br />

On or about May 8, 2015, the hospital pharmacy<br />
discovered a narcotic discrepancy, which left five<br />
5mg oxycodone IR tablets missing. The pharmacy<br />
transaction report showed that Respondent was the only<br />
staff member who had accessed the drawer containing<br />
the oxycodone tablets. Respondent had administered<br />
three 5 mg oxycodone IR tablets to patients. Two 5 mg<br />
oxycodone IR tablets were still unable to be accounted for<br />
and Respondent was unable to explain the discrepancy.<br />
Respondent denied diverting the missing medication.<br />
During questioning, hospital personnel observed<br />
Respondent’s eyes were cloudy, her gait was unsteady,<br />
she seemed confused by the questions, and her speech<br />
was slurred. Respondent refused to submit to a for-cause<br />
drug screen and attempted to climb out a window in order<br />
to leave the premises. Respondent ultimately left the<br />
premises without submitting to the for-cause drug screen.<br />

The <strong>Missouri</strong> <strong>State</strong> <strong>Board</strong> <strong>of</strong> <strong>Nursing</strong> received information<br />
from the Kansas <strong>Board</strong> <strong>of</strong> <strong>Nursing</strong> via the NURSYS<br />
website that the nursing license <strong>of</strong> Respondent was<br />
revoked in a Proposed Default Order dated July 27, 2016.<br />

An audit performed in <strong>August</strong> 2015 showed that patient<br />
S.A. only received pain medication when Respondent was<br />
her nurse. Patient S.A. had not received pain medication<br />
in the previous days when other nurses were assigned to<br />
her care. Respondent documented that she administered<br />
two tablets <strong>of</strong> Norco to patient S.A. on <strong>August</strong> 17, 2015 at<br />
12:00 p.m. and 6:00 p.m. and two more tablets around 8:00<br />
a.m. on <strong>August</strong> 18, 2015. Respondent did not document<br />
patient S.A.’s pain level on these days. A drug screen<br />
was performed on Patient S.A. on <strong>August</strong> 18, 2015, at<br />
1:00 p.m. and the results were negative. The records also<br />
noted that Respondent had administered a tablet <strong>of</strong> Norco<br />
to Patient M.W., who was not assigned to Respondent’s<br />
care, thirty minutes after patient M.W. had received a PCA<br />
bolus. Patient M.W. had undergone surgery the same day.<br />
Further, Respondent documented the administration <strong>of</strong><br />
two tablets <strong>of</strong> Norco at 9:00 a.m. on <strong>August</strong> 17, 2015, to<br />
patient J.W.; however, subsequently, another nurse found<br />
this medication in a cup in patient J.W.’s room. When<br />
questioned, patient J.W. stated she had received one tablet
