All Care’s Adult Foster Care Program
provides a better alternative for families
All Care’s Adult Foster Care (AFC) program is
a wonderful alternative to a nursing home or assisted
living facility for anyone who is disabled and/
or chronically ill and has access to MassHealth. The
program helps to provide dignity and independence
while improving quality of life for individuals who
require round the clock supervision, 7 days a week.
Here’s just one of the many success stories from
our AFC program. Kathleen had been living independently
in her own apartment for many years
but as her disease progressed, she experienced
multiple hospitalizations. Her last hospitalization
resulted in a short term stay in a rehabilitation
hospital. During this time, Kathleen experienced
increased cognitive deficits and had difficulty caring
for herself without assistance. It was recommended
that Kathleen stay in the facility and
permanently reside on the Long-Term Care Unit.
Kathleen was very unhappy about this. Fortunately,
the facility’s social worker spoke to the family
about the option of Kathleen returning home with
the assistance of one of her children through All
Care’s Adult Foster Care Program. Kathleen’s
daughter, Marie, moved in with her mother and
now provides the care that Kathleen needs while

receiving a tax-free stipend twice a month through
MassHealth to help ease the financial burden of
caring for her Mom.
Many AFC clients were already living with a
parent, child or other family member who qualifies
as a caregiver. Others are placed in homes with
compassionate caregivers who are special, dedicated
people who provide meals, companionship and
personal care assistance to our AFC clients who
do not have families close by. The AFC program
also provides monthly nursing and social work visits
where both the client and caregiver receive the
support and resources they need to share a safe,
happy, fulfilling life together.
Since she has been at home, Kathleen’s appetite
and memory have improved and she has remained
out of the hospital. Kathleen is now able to spend
quality time with her grandchildren and enjoy meals
in the comfort of her own home. All Care’s nurses
and social workers visit the family monthly to provide
support and ensure that the home remains a
positive, safe and stable environment for everyone.
Clients who are enrolled in our AFC program
also have access to our Visiting Nurse Association,
Hospice, and Private Duty divisions, which
the case manager can access as needed to ensure
the individual receives the full range of services
to stay at home with safety and comfort. If you or
