Health Care Power of Attorney and Living Will forms are available at the Senior Centers.<br />

Forms must be witnessed and notarized, which can be done for free at the<br />

Robert and Pearl Seymour Center in Chapel Hill, the Central <strong>Orange</strong> Senior Center<br />

in Hillsborough, or at your bank.<br />

It is advisable to seek an attorney when making a will, establishing a trust or designating<br />

a Durable or Financial Power of Attorney to make property, financial and other legal<br />

decisions.<br />
