II. Research, Teaching, Clinical<br />
A. Research:<br />
My research centers on female pelvic geometry and soft tissue biomechanics. The majority<br />
of women who undergo vaginal childbirth do not develop pelvic floor dysfunction (PFD).<br />
Similarly, some women who undergo childbirth develop pelvic muscle damage, and others<br />
do not. There are a number of reasons for this, but based on my observations, one may<br />
involve the bony pelvic geometry and its interaction with the levator muscles and pelvic<br />
soft tissues. My research studies the impact of bony pelvic geometry on childbirth related<br />
pelvic soft tissue injury, and the relationship between pelvic soft tissue injury and pelvic<br />
floor dysfunction. I use magnetic resonance imaging (MRI), MR-based three-dimensional<br />
modeling, and structural analysis of the pelvic floor. The knowledge gained will help to<br />
identify those nulligravidas at risk for childbirth-related pelvic tissue injury. This risk will<br />
likely be modulated by maternal, fetal and labor parameters as well as collagen and tissue<br />
genetics. My research also uses MRI and 3D reconstruction techniques to study changes in<br />
the levator ani and bladder neck after childbirth. This is an ancillary analysis of data from<br />
the NIH sponsored Pelvic Floor Disorders Network, comparing pelvic anatomy after<br />
cesarean and vaginal delivery in 250 primiparas. This work evaluates the impact of bony<br />
pelvic geometry and labor parameters on the pelvic floor structures.<br />
I am also studying racial differences in the levator ani muscles, to evaluate the possible<br />
impact on differential rates of PFD across racial groups. Further, I am collaborating with<br />
structural engineers to develop MR-based pelvic models that can be used to simulate<br />

childbirth, and study the impact of bony shape and muscle characteristics on childbirth<br />
injury to the pelvic floor.<br />

C. Report of Current Research<br />
My current research is focused on MR based pelvic models for childbirth simulation, as<br />
well as on post partum pelvic floor changes after vaginal and cesarean birth. I am presently<br />
leading repeatability/reliability studies to determine the stability of MR based 3D<br />
reconstruction and measurement of the female pelvic floor structures.<br />

E. Report of Clinical Activities:<br />
My clinical activities consist of a hospital based consultative service for female pelvic floor<br />
disorders and childbirth related injuries. Eighty percent of my time is committed to clinical<br />
practice, involving office and operating room based clinical care of women with complex<br />
pelvic floor disorders.<br />
I also direct the interdisciplinary pelvic floor disorders group at Tampa General Hospital.<br />
The group is comprised of pelvic floor specialists from Colorectal surgery, Urology,<br />
Urogynecology, pelvic pain, physical therapy, and mental health. The group meets<br />
regularly to discuss complex pelvic floor dysfunction cases, and collaborate on treatment of<br />
these patients.<br />
Through my collaboration with colleagues in radiology, I was the first gynecologist to<br />
develop 3D pelvic floor MRI and introduce it into gynecological research and clinical<br />
practice. This has since evolved into 3D pelvic floor ultrasound, which is gaining usage in<br />
research and clinical practice.<br />
