A) Una intubación fácil implica una <strong>extubación</strong> sin<br />
complicaciones.<br />
B) <strong>La</strong> necesidad <strong>de</strong> reintubación es más probable<br />
tras cirugía <strong>de</strong> cabeza y cuello.<br />
C) Una <strong>vía</strong> <strong>aérea</strong> <strong>difícil</strong> es aquel<strong>la</strong> situación en <strong>la</strong><br />
que un anestesiólogo experimentado tiene dificultad<br />
para <strong>la</strong> venti<strong>la</strong>ción con mascaril<strong>la</strong> y/o<br />
para <strong>la</strong> intubación traqueal.<br />
D) <strong>La</strong> inci<strong>de</strong>ncia <strong>de</strong> "<strong>extubación</strong> fallida" es más<br />

B) El efecto residual <strong>de</strong> re<strong>la</strong>jantes muscu<strong>la</strong>res.<br />
C) El nivel <strong>de</strong> consciencia.<br />
D) <strong>La</strong> posición durante <strong>la</strong> intervención quirúrgica.<br />
E) <strong>La</strong> existencia <strong>de</strong> una insuficiencia respiratoria<br />
hipoxémica.<br />
3) Cuando p<strong>la</strong>neamos <strong>la</strong> <strong>extubación</strong> <strong>de</strong> un paciente<br />
que ha presentado una dificultad <strong>de</strong> intubación imprevista,<br />
<strong>la</strong>s opciones a elegir son:<br />
A) Extubación con guía <strong>de</strong> intercambio <strong>de</strong> TET<br />
B) Realizar traqueostomía y posterior <strong>extubación</strong>.<br />
C) Extubación con el paciente anestesiado cuando<br />
aún no haya recuperado reflejos para evitar estimu<strong>la</strong>ción<br />
<strong>la</strong>ringotraqueal.<br />
D) Extubación guiada con fibrobroncoscopia.<br />

E) Extubación y realización <strong>de</strong> venti<strong>la</strong>ción jet<br />
transtraqueal si <strong>la</strong> venti<strong>la</strong>ción/oxigenación es<br />
ineficaz.<br />
4) Para preparar <strong>la</strong> <strong>extubación</strong> <strong>de</strong> un paciente con <strong>vía</strong><br />
<strong>aérea</strong> <strong>difícil</strong> conocida es necesario:<br />
A) Tener disponible y bien equipado el carro <strong>de</strong><br />
intubación y preparar previamente el material<br />
que vayamos a necesitar.<br />
B) Desinf<strong>la</strong>r el neumobalón lentamente sin aspirar<br />
orofaringe para evitar tos y <strong>la</strong>ringoespasmo.<br />
C) Realizar traqueostomía urgente si el paciente<br />
presenta e<strong>de</strong>ma subglótico post<strong>extubación</strong>.<br />
D) Es indispensable colocar un catéter <strong>de</strong> intercambio<br />
<strong>de</strong> TET como paso intermedio para asegurar<br />
una <strong>extubación</strong> sin inci<strong>de</strong>ncias.<br />
E) Preoxigenar al paciente y realizar vigi<strong>la</strong>ncia estrecha<br />
<strong>de</strong> sus constantes vitales y saturación <strong>de</strong> O 2.<br />
5) <strong>La</strong> <strong>extubación</strong> es una extensión lógica <strong>de</strong> <strong>la</strong> estrategia<br />
<strong>de</strong> <strong>la</strong> intubación y se recomienda:<br />
A) Una preparación <strong>de</strong> <strong>la</strong> misma <strong>de</strong> acuerdo con <strong>la</strong>s<br />
condiciones <strong>de</strong>l paciente y el tipo <strong>de</strong> cirugía realizada.<br />
B) El método elegido para llevar<strong>la</strong> a cabo <strong>de</strong>pen<strong>de</strong><br />
en gran medida <strong>de</strong> los medios disponibles.<br />
C) <strong>La</strong> <strong>extubación</strong> se realizará <strong>de</strong> acuerdo con <strong>la</strong>s<br />
preferencias o habilida<strong>de</strong>s <strong>de</strong>l anestesiólogo responsable.<br />
D) Reevaluar siempre mediante pruebas complementarias<br />
(TC, RM, arteriografía...) el estado <strong>de</strong><br />
<strong>la</strong> <strong>vía</strong> <strong>aérea</strong> al final <strong>de</strong> <strong>la</strong> cirugía si consi<strong>de</strong>ramos<br />
que <strong>la</strong> <strong>extubación</strong> será dificultosa.<br />
E) Consi<strong>de</strong>rar toda <strong>extubación</strong> como potencial reintubación.<br />
