Asimismo, en pacientes con bajo riesgo<br />
hemorrágico y alto riesgo isquémico<br />
se ha explorado el posible beneficio de<br />
utilizar una DAPT extendida 26 .<br />
Los estudios clínicos han hecho un<br />
esfuerzo por identificar la proporción<br />
riesgo/beneficio de la DAPT prolongada<br />
dependiendo el perfil de riesgo del<br />
paciente, generando herramientas de<br />
puntajes para predecirlo: actualmente<br />
el más utilizado es el “DAPT score”<br />
que tiene como variables edad,<br />
tabaquismo, diabetes, ICP o infarto<br />
previo, presentación del infarto, diámetro<br />
del stent, si se trata de stent liberador de<br />
paclitaxel®, función ventricular y puente<br />
de vena safena, un puntaje mayor a 2<br />
se ha asociado con mayor beneficio de<br />
DAPT prolongada (Tabla 1) 26 .<br />
El advenimiento de nuevas tecnologías de<br />
imagen invasiva así como los dispositivos<br />
para la realización de ICP nos ayudan a<br />
mejorar nuestro beneficio clínico neto en<br />
la constante y dinámica individualización<br />
de la terapia cardiovascular.<br />
