Enfin, la troisième évolution est l’émergence de l’intelligence artificielle et l’intégration
des big data au robot opératoire. L’idée est que les machines ‘apprennent’
en exploitant toutes les big data obtenues sur le patient. Un scan peut par
exemple donner des images 3D et en réalité virtuelle de l’anatomie du patient.
On peut ainsi obtenir de meilleurs résultats lors des interventions complexes.
Dans l’émission ‘Topdokters’, on a par exemple pu voir le professeur Karel
Decaestecker de l’UZ Gent (voir encadré) pratiquer l’ablation d’une tumeur rénale
avec le robot opératoire. Pressé par le temps, il ne disposait que de 20 minutes
pour réséquer la tumeur, car le rein se nécrose lorsque l’artère est clampée trop
longtemps. Grâce à la réalité augmentée, le chirurgien peut visualiser la situation
et les problèmes en temps réel.

Where is robotic surgery t oday?
The US market for robotic surgery has been growing at an impressive rate since inception,
with growth of 14 % and 17% year over ye ar for the previous two years for Intuitive. The
company reported 753,000 procedures in the US alone in 2018, with guidance of 15 -17%
in worldwide procedure growth expected in 2019. Judging by the maintenance of these
here is no question we are still in early stages for
robotics. Specifically when taking a look at the trailing 5 -year growth rates for these
procedures categories we can see general surgery has been growing an impressive 30 -
35% year over year (chart 2) . Se veral features of the technology can be attributed to
driving the growth we’ve seen in recent years, most of which have been described as
making surgeon’s life easier in the operating suite. Back tracking to the 1960s and 70s we
saw the rise of laparoscopi c surgery in gynecology due mainly to enhanced surgical access
to the pelvic cavity. In the late 1980s we finally saw this technology utilized in the
abdomen for general surgery procedures. Within general surgery, numbers though, the at an drive increasing was larger base t
less about ergonomics, but rather improved clinical outcomes from smaller surgical
incisions. Large open abdominal incisions are associated with increased morbidity and
mortality from surgical site infection, wound dehiscence, ventral hernia, and post -
operative pain, among others. Laparoscopic surgery has helped to mitigate many of these
post-op complications, but if it has been so successful why robotics?

big data est extrêmement vaste. Une étude de Bank of America Merrill Lynch
a révélé que 21 millions d’interventions chirurgicales étaient réalisées chaque
année rien qu’aux États-Unis. Dans son analyse, elle conclut qu’on pourrait à
terme pratiquer jusqu’à 6 millions d’interventions assistées par la robotique par
an aux États-Unis. En 2018, le taux de pénétration n’était que de 10 à 20%. Plus
de 60% des interventions réalisées dans le monde où un robot pourrait être mis
en œuvre se pratiquent toujours par chirurgie ‘ouverte’ classique. Sur les moins
de 40% d’opérations réalisées par une technique mini-invasive, on estime que
moins de 5% se font avec un robot. Le potentiel de croissance est donc énorme.

From our experience speaking with surgeons and attending the SRS conference it is clear
that there are many pain points that robotics has help to address in the OR. Visualization
has of course always been a limitation in abdominal and pelvic surgery. Yes, laparoscopy
has made tremendous bounds here, but the imaging capabilities between lap and robotic
systems hardly compare today. With lap you have an excellent high-definition camera
attached to the end of the scope that projects to video screens in the OR. The problem is
