Differential Diagnosis:<br />
the start of major mental disturbances, in particu<strong>la</strong>r in the<br />
excited phase of bipo<strong>la</strong>r affective disorder and during<br />
schizophrenic decompensations.<br />
In bipo<strong>la</strong>r mania, however, sleep is also typically<br />
shortened, and such patients have no difficulty arising at<br />
a conventional hour.<br />
In addition, the de<strong>la</strong>yed sleep pattern is usually transient<br />
in major mental disorders and covaries with the mental<br />
symptoms in severity over time.<br />
Persistence of phase-de<strong>la</strong>yed sleep after remission of the<br />
mental symptoms suggests that the patient has DSPS<br />
that was unmasked by the initial mental disturbance.
