Das sexuelle Profil eines Paares hat, wie die<br />
systemische Sexualtherapie zeigt, oft ein ungeahntes<br />
Entwicklungspotential. In der Therapie<br />
geht es u.a. darum, beide Partner zu<br />
ermutigen, ihr Sexualleben – wie häufig zu<br />
Beginn der Partnerschaft – wieder phantasievoll<br />
und risikoreicher zu gestalten. Dabei<br />
spielt die Bearbeitung von Selbstzweifeln und<br />
Zweifeln gegenüber dem Partner eine wichtige<br />
Rolle, da diese Phantasie und Humor lähmen<br />
und Sex zu einem langweiligen Phänomen verkümmern<br />
lassen. Manche Paare sind in der<br />
Lage, Selbstzweifel mutig hinter sich zu lassen,<br />
andere trennen und scheiden sich und inszenieren<br />
in neuen Beziehungen wieder dieselbe<br />
Komödie oder Tragödie. n<br />

Consultation:<br />
A positive multidisciplinary approach<br />
in pregnant women<br />
Educational Objective:<br />
n Participants learn how different disciplines<br />
can organize effective care in pregnant women<br />
with psychiatric disease and/or psychosocial<br />
problems and why this is important.<br />
Purpose:<br />
Pregnant women with psychiatric diseases,<br />
and/or use of psychotropic medication and, in<br />
particular, women with psychosocial problems<br />
(e.g. homelessness, low intelligence,<br />
teenage pregnancy, drug abuse, physically<br />
and/or sexually abuse etc.) face increased maternal,<br />
foetal and neonatal risks which cannot<br />
be managed by a midwife or obstetrician<br />
alone.<br />
Methods:<br />
In several General Teaching Hospitals in The<br />
Netherlands, a system of multidisciplinary<br />
care has been developed, in which a multidisciplinary<br />
team of psychiatrist, psychologist,<br />
obstetrician, paediatrician, social worker,<br />
teenage pregnancy counsellor, midwife, pedagogue<br />
and mental health care nurse formulate<br />
and practice comprehensive guidance<br />
and treatment modalities. Preferably, if possible,<br />
this counselling is done before pregnancy.<br />
The example of Gelre Teaching Hospital<br />
Apeldoorn will be explained.<br />
Results:<br />
All partners in care, and the pregnant woman<br />
herself and her partner, if present, know when<br />
and what is necessary to do. Paediatricians<br />
know in advance which newborn babies are at<br />
potential psychosocial risk. Video home training<br />
for babies is used to improve bonding<br />
and attachment between the parent(s) and the<br />
baby. In more complicated cases, Infant Mental<br />
Health is used to improve parent-baby interaction.<br />
Health care givers in the whole<br />
chain have become more sensitive in recognizing<br />
pregnant women at risk.<br />
Conclusions:<br />
This multidisciplinary initiative has improved<br />
the healthcare for pregnant women with psychiatric<br />
diseases and women with psycho -<br />
social problems. Cross-border consultation<br />
results in more effective healthcare and improved<br />
care for the babies born in this project.<br />
