Konsequenzen für die Praxis<br />
Sehr geehrte Kolleginnen und Kollegen,<br />
liebe Freunde der <strong>Neue</strong>n <strong>Gruppe</strong> !<br />
Bekanntlich führen viele Wege nach Rom, sichere und unsichere. Man könnte auch behaupten,<br />
in ähnlicher Weise gibt es zahlreiche therapeutische Verfahren, Techniken und Materialien auf<br />
dem Weg zum Ziel, zu einer erfolgreichen Parodontalbehandlung.<br />
Welche Methode, Technik und welches Material jedoch wann und auf welche Art indiziert ist,<br />
ist nach wie vor umstritten. Die Diskussionen darüber werden seit Jahren kontrovers zum Teil<br />
leidenschaftlich geführt, mit vielen Veröffentlichungen, Statements und Empfehlungen.<br />
Eine sorgfältige Datenanalyse, Langzeitergebnisse und vor allem eine klare Bewertung und<br />
Einordnung, die von Unabhängigkeit geprägt ist, sind für die Praxis von entscheidender<br />
Bedeutung. Wir benötigen tagtäglich das sichere Gefühl einer bestens abgesicherten Therapie-<br />
Entscheidung mit der Aussicht auf den größtmöglichen Erfolg unserer Parodontalbehandlung.<br />
Schließlich stehen wir in der langjährigen Verantwortung unserem Patienten gegenüber.<br />
Vor diesem Hintergrund soll unsere Jahrestagung 2005 in Hamburg mit ihren renommierten<br />
Referenten Ihnen, liebe Kolleginnen und Kollegen, ein bedeutsamer Wegweiser für Ihr tägliches<br />
Tun werden und Ihre Entscheidungsfindung erleichtern.<br />
Ohne den Einsatz unserer qualifizierten Mitarbeiterinnen wird unser Behandlungserfolg nur ein<br />
temporärer sein. Daher geben die die Tagung begleitenden Seminare aktuelle Orientierung<br />

Jens Christoph Türp studierte Zahnmedizin in Freiburg i. Br., wo er auch promoviert wurde und<br />

Seit Juni 2001 ist er als Oberassistent (Oberarzt) in der Klinik für Rekonstruktive Zahnmedizin<br />

Evidenzbasierte Zahnmedizin – Maßstab für die Praxis?<br />
Anliegen der evidenzbasierten Zahnmedizin (EbZ) ist es, patientenorientierten Entscheidungen<br />
eine wissenschaftlichere Grundlage zu geben, als dies oft der Fall ist.<br />
Die klinische Praxis der EbZ beinhaltet die Integration von klinischer Expertise (=interner<br />
Evidenz), Patientenpräferenzen und externer Evidenz aus systematischer patientenorientierter<br />
Forschung. Die Parodontologie hat sich in den letzten Jahren in besonders lobenswerter<br />
Weise hervorgetan, um den Gedanken einer EbZ mit Leben zu füllen.<br />
Allerdings werden in zahnärztlichen Praxen häufig Vorbehalte laut. Dies ist insbesondere dann<br />
der Fall, wenn – durch das psychologische Phänomen der Bestätigungstendenz in ihrem<br />
Bestand gefestigte – persönliche Vorlieben tangiert werden. Kollisionen mit geltenden zahnärztlichen<br />
Gebührenordnungen führen ebenfalls dazu, dass neue wissenschaftlich unter -<br />
mauerte Verfahren keine Anwendung finden.<br />
Dennoch bestehen gute Gründe für die Annahme, dass sich die EbZ allmählich durchsetzen<br />
wird – in den Universitäten wie in den Praxen.<br />

Füllermaterialien in der Parodontologie – ist ein Vorteil gegenüber<br />
dem alleinigen Membranverfahren zu erwarten ?<br />
Deep intra-osseous defects represent a major challenge for the clinician. Sites with intraosseous<br />
lesions have been shown to be at higher risk of disease progression in subjects who<br />
had not received systematic periodontal therapy. Reconstructive procedures have been used<br />
with varying success during the past decades to accomplish the restitutio ad integrum of lost<br />
attachment apparatus in deep intraosseous defects. One of the most investigated method to<br />
achieve the reconstruction of intraosseous defects is to combine the access surgery with the<br />
placement of bone grafts or implant biomaterials into the debrided bony lesion with the goal<br />
to regenerate the lost periodontal tissues. Grafting biomaterials include autogenous grafts,<br />
allogenic grafts, xenogenic grafts and alloplastic materials. The assumption behind the clinical<br />
use of GPs is that the complete regeneration of the attachment apparatus (including new<br />
bone formation and new connective tissue attachment) would be enhanced by the various<br />
biomaterials due to their osteogenetic potential (if the graft would contain viable bone- forming<br />
cells), osteinductive capacities (exerted by the release of bone-inducing substances), or<br />
osteoconductive properties (i.e. the possibility to create a scaffold to support bone formation).<br />
Overall, observational and controlled trials have generally reported an additional benefit in<br />
terms of soft and hard tissue improvements following the use of GPs. However, due to the<br />
large variety of graft biomaterials proposed on the market, the magnitude of such improvements<br />
as well as the consistency of the advantage achieved when GPs are compared to an<br />
access flap procedure need to be determined yet. The presentation will focus on the effect<br />
of the use of grafting procedures in addition to conventional open flap debridement in the<br />
treatment of deep intraosseous defects. Specific advantages of the grafting procedures when<br />
compared to membrane-supprtive reconstruction will also be analyzed.<br />

Dr. Mario Roccuzzo<br />
Mario Roccuzzo graduated cum laude from the School of Dental Medicine of Torino in 1987.<br />
He attended the department of Periodontology at the Karolinska Institut in Stockholm and<br />
then completed a postgraduate program in Periodontology at the University of Siena. He has<br />
done extensive research in the field of Periodontal Plastic Surgery and Guided Bone Regeneration.<br />
He has been a clinical instructor and lecturer in Periodontology at the University of Torino.<br />
He is currently a faculty member, teaching Periodontics, in the Division of Oral & Maxillofacial<br />
Surgery.<br />
Mario Roccuzzo is an active member of the Italian Society of Periodontology, fellow of the<br />
International Team for Implantology (ITI), chairman of the ITI Italian section. He has lectured<br />
extensively in Europe, Russia and United States. He maintains a private practice limited to<br />
Periodontology and Implantology in Torino.<br />
Parodontal-chirurgische Verfahren zur Rezessions-Deckung –<br />
Entscheidungskriterien für die Wahl der Methode<br />
Aesthetics is an inseparable part of today’s oral therapy, and several procedures have been<br />
proposed to preserve or enhance patients’ smile. One of the most frequent indications of<br />
perio-plastic surgery is the treatment of buccal gingival recessions. Clinicians, however, have<br />
such a myriad of options for root coverage that treatment choice is not always easy. The evidence<br />
for root coverage efficacy of guided tissue regeneration (GTR), free gingival graft (FGG),<br />
connective tissue graft (CTG), and coronally advanced flap (CAF), has been investigated in the<br />
dental literature and will be presented. Even though no single treatment can be considered<br />
superior to all the others, CTG was statistically significantly more effective than GTR in recession<br />
reduction. Attention will be devoted to illustrate factors most associated with successful<br />
outcomes. Several cases will be presented to illustrate when certain root coverage procedures<br />
are indicated and when they are not.<br />

Prof. Dr. Andrea Mombelli<br />
Professor Mombelli ist Ordinarius für Parodontologie und Orale Pathophysiologie an den Zahn -<br />
medizischen Kliniken der Universität Genf (Schweiz). In dieser Funktion ist er verantwortlich für<br />
die Vor- und Nachdiplomausbildung im Fach Parodontologie. Er ist außerdem Präsident der<br />
Zahnmedizinischen Sektion der Medizinischen Fakultät und Präsident der Schweizerischen<br />
Gesellschaft für Parodontologie. Professor Mombelli hat an der Universität Bern Zahnmedizin<br />
studiert. 1979 erfolgte die Promotion, 1988 die Ernennung zum Spezialisten in Parodontologie<br />
SSP/SSO und 1992 die Habilitation im Fach Zahnmedizin (speziell Parodontologie und Orale<br />
Mikrobiologie). In der Forschung befasst er sich hauptsächlich mit mikrobiologischen, immunologischen<br />
und klinischen Aspekten parodontaler und periimplantärer Infektionen.<br />
Sein wissenschaftliches Werk umfaßt über 130 Publikationen, davon sind über die Hälfte<br />
Originalarbeiten in internationalen Fachzeitschriften. Als Referent ist Professor Mombelli regelmäßig<br />
im Inland, in Europa und Übersee tätig. Für seine wissenschaftlichen Arbeiten hat<br />
Professor Mombelli mehrere Preise gewonnen. Unter anderem ist er Honorarprofessor der<br />
Universität Buenos Aires und Ehrenmitglied der Belgischen Gesellschaft für Parodontologie.<br />
Chirurgische versus nicht-chirurgische Therapie –<br />
vom Dogma zur Wissenschaft<br />
Parodontalpathogene Keime in der subgingivalen Plaque sind die Ursache der Parodontitis.<br />
Ihre Entfernung muss das primäre Ziel der Parodontalbehandlung sein. Die Hauptmasse dieser<br />
Bakterien befindet sich in einem Biofilm, der auf der Wurzeloberfläche haftet. Die gründliche<br />
mechanische Depuration aller bakteriell kontaminierten Zahnoberflächen und eine gute<br />
Mundhygiene nach der Behandlung sind die entscheidenden Faktoren für den Erfolg der<br />
Therapie, ob chirurgisch oder nicht. Mittels umfangreicher klinischer Langzeitstudien konnten<br />
weitere Faktoren identifiziert werden, die das Resultat einer Parodontalbehandlung beeinflussen,<br />
oder das Risiko für ein Wiederaufflammen der Erkrankung erhöhen. Dazu gehören veränderbare<br />
und nicht veränderbare Faktoren auf der Ebene des Patienten, z.B. Tabakrauchen,<br />
und genetisch determinierte Prädispositionen, sowie lokale Faktoren, die die behandelten<br />
Stellen betreffen. Sowohl die nicht-chirurgische als auch die chirurgische Therapie haben ihre<br />
Limiten und eventuell unerwünschte Nebenwirkungen. Die nicht-chirurgische Vorgehensweise<br />
scheint auf den ersten Blick weniger traumatisch zu sein, und ist tatsächlich bis zu einer<br />
gewissen Taschentiefe im Mittel vorteilhafter. Bei erschwertem Zugang kann jedoch eine<br />
chirurgische Intervention effizienter und schonender sein als multiple nichtchirurgische<br />
Behandlungsversuche. Klinische Studien zeigten denn auch, dass bei chirugischer Vorgehensweise<br />
in tiefen Taschen Konkremente effizienter entfernt und Taschen stärker reduziert wurden<br />
als bei nicht-chirugischer Behandlung. Darüber hinaus bedingen regenerative Methoden den<br />
chirurgischen Zugang.<br />

He is Past-President of the Italian Society of Periodontology and of the European Federation of<br />

Periodontology and Implant Dentistry” J. Lindhe, T. Karring, N.P.Lang Editors.<br />
He limits his activity to Periodontology in a Specialty practice in Rome and in a Group Practice<br />

The Furcation Problem. Resective Therapy<br />
An analysis of risk factors at tooth and at site level include the presence of furcated molars as<br />
a 13 fold risk of tooth loss compared to molars with intact furcations. Several therapeutical<br />
approaches have been successfully utilised to treat these lesions and to diminish the risk of<br />
tooth loss. One of the these, is the resective approach which has the objective of the physical<br />
elimination of the horizontal component of the interradicular defect or of the root itself.<br />
Several considerations must be taken into account to be successful with this therapy.<br />
1) Indications and contraindication on the bases of anatomical variations.<br />
2) Factors which might influence the selection of a particular therapy.<br />
3) Long term results.<br />
4) Situations where the failure risk is increased with respect to the mean values.<br />
5) Therapeutic protocol as a guide to maximise success.<br />

The Furcation Involvement - A Problem in Treatment Planning<br />
Regenerative Therapy Following Furcation Invasion<br />
One of the greatest challenges in periodontics continues to be the treatment of multirooted<br />
teeth demonstrating inter-radicular loss of the periodontium (furcation invasion). While added<br />
stability maybe provided by extra root anchorage, furcated teeth and their surrounding tissues<br />
make treatment difficult and results unpredictable with current therapies. Although there are<br />
reports of clinical success in the treatment of Class II furcations, confirmation by histologic<br />
documentation of new bone, periodontal ligament and cementum on a root surface previously<br />
exposed to disease has not been available. New information produced by treating<br />
angular defects with a variety of materials has now been used to treat Class II furcations. The<br />
application of the priniciples of tissue engineering with purified growth factors appears to<br />
have greatly increased the likelihood of achieving this elusive goal. This presentation will compare<br />
previous efforts of regeneration with contemporary treatment regimes.<br />

Was ist evidenzbasiert?<br />
Das Ziel regenerativer Parodontaltherapie ist die Wiederherstellung des verlorenen Zahnhalte -<br />
apparates (d.h. die Neubildung von Wurzelzement, Desmodont und Alveolarknochen). Ergebnisse<br />
aus der Grundlagenforschung haben auf die wichtige Rolle der Schmelz-Matrix-Proteine<br />
(Enamel-Matrix-Proteins kurz EMD) in der parodontalen Wundheilung hingewiesen. Histologische<br />
Ergebnisse aus Tierversuchen und aus einigen humanen Fallberichten konnten zeigen,<br />
dass die Behandlung mit EMD die Neubildung des Zahnhalteapparates fördert. Des Weiteren<br />
konnte in klinischen Studien gezeigt werden, dass die Behandlung mit EMD die parodontale<br />
Wundheilung am Menschen positiv beeinflusst. Das Ziel des Vortrags ist, basierend auf die<br />
vorhandene Evidenz die klinischen Anwendungsbereiche für die Schmelz Matrix Proteine<br />
darzustellen.<br />

Born and raised in Genova, Italy, Maurizio Tonetti did his specialty training in Periodontology<br />
at the Harvard School of Dental Medicine. Started a private practice limited to Periodontology<br />
and Implant Dentistry in Genova, Italy and moved to Berne Switzerland where he was active<br />
as assistant and then associate Professor in the Clinic for Periodontology and Fixed Prosthodontics<br />
directed by Prof Lang. In 1999 he became Head of the Department of Periodontology<br />
at the Eastman Dental Institute and Hospital, University College London. There he established<br />
a premier specialist training program in Periodontology and a strong research activity. After 5<br />
year of tenure he moved to the United States and currently serves as the Head of the Department<br />
of Oral Health and Diagnostic Sciences and Chair of the Division of Periodontology at<br />
the School of Dental Medicine of University of Connecticut, one of the top 3 American dental<br />
schools. He is the Editor in Chief of the Journal of Clinical Periodontology. Has published more<br />
than 100 original scientific articles and is credited with significant changes in the scientific<br />
understanding and clinical management of periodontitis.<br />
GTR – Verfahren versus traditionelle chirurgische Behandlung<br />
bei intraossären Defekten<br />
There is considerable evidence that regenerative surgery brings significant added benefits in<br />
terms of increase in tooth support, resolution of deep pockets associated with intrabony<br />
defects and change in tooth prognosis. This presentation will focus on a systematic assessment<br />
of the evidence base of these statements but will also cover the practical implications arising<br />
from this knowledge. Focus will be on the decision making strategies to achieve optimal<br />
results and high levels of predictability. Finally the current knowledge related to the relative<br />
performance of teeth with regenerated ligmanent and an implant based alternative will be<br />
discussed.<br />

Prof. Dr. Marc Quirynen<br />
Professor M. Quirynen graduated in 1980 as dentist at the Catholic University of Leuven and<br />
finished in 1984 his training in periodontology at the department of Periodontology (Catholic<br />
University Leuven). In 1986 he presented his Ph. D. entitled: "Anatomical and inflammatory<br />
factors influence bacterial plaque growth and retention in man”. In 1990 he was appointed<br />
professor at the Faculty of medicine of the Catholic University of Leuven to teach periodontology<br />
and anatomy. His research deals mainly with oral microbiology, with special attention to<br />
the influence of surface characteristics on bacterial adhesion and the effect of antiseptics. He<br />
published over 200 full papers in international peer-reviewed journals. He is member of the<br />
editorial board of the Journal of Clinical Periodontology (associate editor), Clinical Oral Implants<br />
Research, Periodontal Practice Today and Parodontologie.<br />
Antibiotics / Antiseptics / New Treatment Strategies for Periodontitis.<br />
The existence of an "intra-oral” transmission of bacteria (from one niche to another, also called<br />
translocation or cross-infection) has recently been thoroughly investigated. Such a transmission<br />
of pathogens, from one locus to another, could of course jeopardize the outcome of periodontal<br />
therapy. The importance/significance of such an intra-oral translocation is difficult to<br />
prove and/or to quantify.<br />
A large series of papers compared the microbiota in pockets around teeth with that in periimplant<br />
pockets in partially edentulous patients and reported a striking similarity. Based on this<br />
similarity, it has been suggested that, at least in partially edentulous patients, teeth act as a<br />
reservoir for the (re)colonization of the subgingival area around implants. This hypothesis was<br />
supported by the observation of similar intra-subject pulsed field gel electrophoresis patterns<br />
for periopathogens from teeth and implants, compared to large inter-subject variations.<br />
In order to reduce the chance for an intra-oral transmission (also called cross-contamination),<br />
a new treatment strategy called "one-stage, full-mouth disinfection" has been introduced. This<br />
strategy attempts to eradicate, or at least suppress, periodontopathogens in a short time span,<br />
and this not only from the periodontal pockets but from all their intra-oral habitats (mucous<br />
membranes, tongue, and saliva). The one-stage full-mouth disinfection concept consists of a<br />
combination of following therapeutic efforts: a full mouth scaling and root planing within 24<br />
to reduce the number of subgingival pathogenic organisms, a subgingival irrigation of all<br />
pockets with a 1% chlorhexidine gel in order to kill remaining bacteria, tongue brushing with<br />
an antiseptic to suppress the bacteria in this niche, mouth rinsing with an antiseptic to reduce<br />
the bacteria in the saliva and on the tonsils. Several comparative studies between the onestage,<br />
full-mouth approach and the standard therapy (root planing per quadrant with 2 week<br />
intervals), clearly illustrated the benefits of such a full-mouth approach in relation to gain in<br />
attach ment, pocket depth reduction, microbiological shifts. A similar approach also resulted in<br />
significant additional improvements during guided tissue regeneration and/or the application<br />
of local antibiotics.<br />
The benefits after systemic use of antibiotics are in general very limited, probably because the<br />
subgingival plaque, in the form of a biofilm, is very resistant. Therefore antibiotics can only be<br />
indicated when this biofilm has been disturbed beforehand. Moreover, the increased risk for<br />
bacterial resistance to antibiotics should preserve their use for more life threatening infections.<br />
Besides pathogens the periodontal pocket also harbours a number of so called beneficial<br />
species. These species could be applied after periodontal therapy to replace the pathogens.<br />
Such an approach, called replacement therapy, seems to be very helpful in other fields of<br />
medicine. Early observations with a replacement therapy in the treatment of periodontitis, both<br />
in vitro as well as in the animal, are very promising.<br />

Prof. Dr. Jörg Meyle, geboren 1953 in Eßlingen, studierte von 1975 bis 1980 Zahnmedizin an<br />
der Eberhard-Karls-Universität Tübingen. 1984 promovierte er dort mit einer Arbeit zur Tastsensibilität<br />
von Zahnimplantaten. Von 1981 bis 1984 erfolgte eine Weiterbildung im Fach<br />
"Zahnärztliche Chirurgie". Ebenfalls in diesem Zeitraum war Prof. Meyle Gast im Forschungslabor<br />
der Abteilung Pädiatrische Hämatologie der Universitätskinderklinik in Tübingen. Von 1981<br />
bis 1994 war er wissenschaftlicher Angestellter der Poliklinik für zahnärztliche Chirurgie und<br />
Parodontologie im Zentrum für Zahn-, Mund- und Kieferheilkunde der Universität Tübingen,<br />
von 1992 bis 1994 in der Funktion eines Oberarztes.<br />
In verschiedenen Forschungsprojekten, die auch von der deutschen Forschungsgemeinschaft<br />
finanziert wurden, beschäftigte er sich auf dem Gebiet der parodontologischen Grundlagenforschung<br />
mit Fragen der Immunzellfunktion bei Zahnbettentzündungen. Über diesen Themenkreis<br />
habilitierte er sich 1992 im Fach Zahn-, Mund- und Kieferheilkunde. Seit Oktober<br />
1994 ist Prof. Meyle Direktor der Poliklinik für Parodontologie am Medizinischen Zentrum für<br />
Zahn-, Mund- und Kieferheilkunde der Justus-Liebig-Universität Giessen.<br />
Als Mitglied in zahlreichen internationalen wissenschaftlichen Vereinigungen wurde er in den<br />
Beirat des "Journal of Clinical Periodontology" und des "Journal of Periodontal Research"<br />
gewählt. Außerdem ist er Mitherausgeber der Zeitschrift "Parodontologie" und "Perio".<br />
Von 1994 – 2003 war Prof. Meyle Mitglied des Vorstandes der Deutschen Gesellschaft für Parodontologie<br />
und von 1998 bis 2002 deren Präsident. Seit 1995 ist er deutscher Delegierter im<br />
Vor stand der European Federation of Periodontology und seit 1998 Mitglied des Exekutivkomitees.<br />
Im Jahr 2001 war er Clinical Visiting Professor im Department of Periodontology, Preventive<br />
Dentistry and Geriatric Dentistry der University of Ann Arbor, Michigan. Von 2001-2003 war er<br />
Leiter des Teams für die Organisation von EUROPERIO 4, und Tagungspräsident. Im Jahr 2003<br />
wurde ihm die Ehrenmitgliedschaft der Deutschen Gesellschaft für Parodontologie verliehen.<br />
Visionen in der Parodontologie<br />
Visionen stehen im Gegensatz zur naturwissenschaftlichen Methode des Messens und<br />
Bewertens, da sie keine solide Basis besitzen. Visionen in der Parodontologie kennzeichnen<br />
daher eine Extrapolation ausgehend vom bisherigen Wissen und aktuellen Erkenntnisstand.<br />
In der parodontalen Diagnostik gibt es viele Entwicklungsmöglichkeiten, die eine differenzierte<br />
Diagnostik verschiedener Erkrankungsformen bei gleichem klinischen Erscheinungsbild<br />
ermöglichen werden. Diagnostische Systeme zum 24-Stunden-Monitoring des parodontalen<br />
Zustandes sind technisch heute schon realisierbar und werden, zumindest die Überwachung<br />
möglicherweise aus der Zahnarztpraxis, auf den häuslichen Bereich verlagern. Mit Hilfe der<br />
schnellen Datenkommunikation könnten diagnostische Systeme etabliert werden, die eine<br />
dauernde Fernüberwachung des Patienten ermöglichen, d.h. er wird nur dann in die Praxis<br />
kommen, wenn ein kritischer Lokalbefund vorliegt.<br />
Therapeutische Systeme werden sich weiter entwickeln, so dass komplexe Methoden unter<br />
Einbeziehung der Zellbiologie und des Tissue-engineerings im großen Stil und mit wesentlich<br />
geringem apparativen und personellen Aufwand zur Verfügung stehen werden.<br />
Ob es einen europaweiten Fachzahnarzt für Parodontologie geben wird und geben muss,<br />
lässt sich nicht schlüssig beantworten, da zukünftig der Schwerpunkt der zahnmedizinische<br />
Ausbildung auf parodontologischem und präventivem Gebiet liegen wird und von daher<br />
gesehen, der Zahnarzt per se eine wesentlich intensivere Spezialausbildung auf parodontologischem<br />
Gebiet haben wird.<br />
Schwer vorhersagbar ist die Beziehung zwischen Parodontologie und anderen Fachdisziplinen.<br />
Verschiedene Szenarien werden unter Einbeziehung wissenschaftlicher, klinischer und<br />
ökonomischer Faktoren vorgestellt und diskutiert werden.<br />

Möglichkeiten und Grenzen der konservativen Parodontalbehandlung<br />
Erfolg in der Parodontaltherapie ist an zwei essenzielle Bedingungen gebunden:<br />
1. Die Ursache der Parodontitis – die subgingivale Plaque – muss vollständig entfernt werden.<br />
2. Eine massive Wiederbesiedlung des behandelten Bereichs muss wirkungsvoll unterdrückt<br />
werden.<br />
Für den Langzeiterfolg müssen beide Bedingungen erfüllt sein: Weder kann eine aufwändige,<br />
rigorose Therapie, oder die zusätzliche Verabreichung von Medikamenten, einen Mangel bei<br />
der Mundhygiene wettmachen, noch kann eine perfekte Mundhygiene den Erfolg garantieren,<br />
wenn die Therapie nie vollständig zuende geführt worden ist. Gerade dieser zweite Punkt<br />
kann für Dentalhygienikerinnen und ZMFs problematisch sein, da ihre Tätigkeit gewisse Grenzen<br />
hat und sie daher bei fortgeschrittenen Fällen nicht immer alles, was für die Gesundung<br />
der Gewebe notwendig wäre, selbst durchführen können. Bei solchen Patienten besteht die<br />
Gefahr, dass die Behandlungsphase nie offiziell beendet wird. Endlose, frustrierende Therapie,<br />
statt echtes Recall, ist die Folge, wobei Attachment und Hartgewebe trotz aller Bemühungen<br />
weiter verloren gehen. In solchen Fällen ist eine detaillierte Re-Evaluation unumgänglich, damit<br />
die notwendigen Zusatzbehandlungen geplant und durchgeführt werden können.

One-stage, Full-mouth Disinfection<br />

The existence of an "intra-oral” transmission of bacteria (from one niche to another, also called<br />
translocation or cross-infection) has recently been thoroughly investigated. In order to reduce<br />
the chance for an intra-oral transmission (also called cross-contamination), the "one-stage, fullmouth<br />
disinfection" has been introduced. This strategy attempts to eradicate, or at least suppress,<br />
periodontopathogens in a short time span, and this not only from the periodontal pockets but<br />
from all their intra-oral habitats (mucous membranes, tongue, and saliva). The one-stage fullmouth<br />
disinfection concept consists of a combination of following therapeutic efforts: a full<br />
mouth scaling and root planing within 24 to reduce the number of subgingival pathogenic<br />
organisms, a subgingival irrigation of all pockets with a 1% chlorhexidine gel in order to kill<br />
remaining bacteria, tongue brushing with an antiseptic to suppress the bacteria in this niche,<br />
mouth rinsing with an antiseptic to reduce the bacteria in the saliva and on the tonsils. Several<br />
comparative studies between the one-stage, full-mouth approach and the standard therapy<br />
(root planing per quadrant with 2 week intervals), clearly illustrated the benefits of such a fullmouth<br />
approach in relation to gain in attachment, pocket depth reduction, microbiological shifts<br />
(for review see Quirynen et al. (2001)). A similar approach also resulted in significant additional<br />
improvements during guided tissue regeneration and/or the application of local antibiotics.<br />
My presentations will give a general overview on the scientific background of the intra-oral<br />
bacterial transmission, as well as of most recent clinical observations with the full-mouth<br />
strategies, including additional improvements of the concept.<br />
Chemotherapeutic Control of Malodour<br />
Breath malodour is a considerable social problem, the incidence of which remains poorly<br />
documented in most countries. Several studies in industrialized countries reported incidences<br />
up to 50%, with a variety degree of intensity. From large scale inventories in multidisciplinary<br />
breath odour outpatient clinics it appears that in the vast majority (85 %) the cause originates<br />
from the oral cavity. Gingivitis, periodontitis and tongue coating are the predominant factors.<br />
Since more than 90% of the population suffers from gingivitis and/or periodontitis, there<br />
is however the risk that such plaque-related inflammatory condition is too easily considered<br />
the cause, while in fact more important pathologies are being overlooked. Indeed, for a minority<br />
of patients extra-oral causes can be identified including: ENT pathology, systemic diseases<br />
(e.g. diabetes, metabolic or hormonal problems, hepatic or renal insufficiency, a bronchial carcinoma<br />
or a gastro-enterologic pathology). A category of subjects imagine they have breath<br />
malodour. This is called imaginary breath odour or halitophobia.<br />
The unpleasant smell of breath mainly originates from volatile sulphide compounds (VSCs).<br />
However, other compounds in mouth air may also be offensive, such as: diamines (e.g. putrescine<br />
and cadaverine), indole, skatole, and butyric or propionic acid. Most of these compounds<br />
result from the proteolytic degradation by oral microorganisms of peptides (sulphide<br />
or non-sulphide containing amino acids) present in saliva, shed epithelium, food debris, gingival<br />
crevicular fluid, interdental plaque, postnasal drip, and blood. Especially gram-negative,<br />
anaerobic bacteria possess such proteolytic activity. When hormonal, gastro-intestinal, renal or<br />
metabolic pathologies are the cause, additional malodorous molecules can be produced<br />
which circulate in the blood and are expressed through the expired air or the crevicular fluid.<br />
Strong antiseptics (e.g. mouth rinses) can reduce oral malodour by killing the bacteria, but of<br />
course have no impact on a bad breath from a non-oral origin. The removal of the substrata<br />
for bacteria (e.g. tongue coating) forms another important treatment strategy, and should not<br />
be forgotten.<br />

Vor- und Nachbehandlung bei chirurgischer Parodontitistherapie<br />
Die chirurgische Therapie ist ein Teilbereich antiinfektiöser Therapiestrategien bei der Behandlung<br />
von Parodontitis. Ziel ist es, am Parodontium Verhältnisse zu schaffen, die im weiteren Verlauf<br />
der Behandlung eine größtmögliche Reduktion parodontalpathogener Mikroorganismen<br />
ermöglichen. Der Parodontalchirurgie sollte immer eine nicht-chirurgische mechanische<br />
Behandlung voraus gehen und eine lebenslange Nachsorgebehandlung folgen. Auf Sicht von<br />
Jahren bestimmt vor allem die Durchführung und Qualität der Nachsorgesorgebehandlung<br />
über Erfolg oder Mißerfolg der Parodontitistherapie.<br />
Wie sollte die mechanische Therapie zur Vorbereitung von parodontalchirurgischen Eingriffe<br />
aussehen? Was schließt die direkte postoperative Behandlung ein? Unterscheidet sich die<br />
Nachsorge nach resektiven, regenerativen oder plastischen Eingriffen? Wie sollte die Nachsorgebehandlung<br />
strukturiert sein?<br />
In dem Vortrag werden die Möglichkeiten und Grenzen der Vor- und Nachsorgebehandlungen<br />
dargestellt und Möglichkeiten aufgezeigt, wie diese Behandlungsschritte in die Praxis<br />
umgesetzt werden können.<br />

Medikamentöse Behandlung der Parodontitis –<br />
Indikationen, Möglichkeiten und Grenzen<br />
Die Parodontologie ist einer der sich am schnellsten weiterentwickelnden Fächer in der Zahnheilkunde.<br />
Insbesondere die adjuvante medikamentöse Behandlung hat in den letzten Jahren<br />
einen höheren Stellenwert bekommen und ist ein essentieller Bestandteil der täglichen<br />
Behandlungsroutine geworden. Bei richtiger Indikation können adjuvante antimikrobielle oder<br />
immunmodulatorische Strategien die Ergebnisse der mechanischen Therapie verbessern. Aus<br />
der Fülle der zur Verfügung stehenden Medikamente ist es jedoch mitunter schwer, die für<br />
den individuellen Patienten bestmöglichen Therapieverfahren auszuwählen. Zum anderen<br />
kommt auch der zeitlichen Koordination der Therapieschritte eine hohe Bedeutung zu.<br />
Der Vortrag gibt eine Übersicht über die aktuellen Produkte der systemischen und lokalen<br />
adjuvanten medikamentösen Therapie bei Parodontitis. Die Einsatzmöglichkeiten, die praktische<br />
Umsetzung und die Grenzen werden in praxisrelevanten Therapiekonzepten dargestellt.

Das "Update" soll neue Entwicklungen und Möglichkeiten bei der Behandlung von Parodontitis -<br />
patienten darstellen, wobei auch Vorteile von Bewährtem nicht außer acht gelassen werden.<br />
Subgingivales Debridement: maschinell oder mit Handinstrumenten?<br />
Wie erreiche ich eine möglichst glatte Wurzeloberfläche? Mit welchen Instrumenten entferne<br />
ich zuverlässig Zahnstein und Biofilm? Es werden bewährte und neue Tendenzen bei der<br />
Handinstrumentation aufgezeigt und die Geräte zur maschinellen Belagsentfernung und<br />
deren Einsatzgebiete dargestellt.<br />
Die Zahnflächenpolitur: Neuheiten und Traditionelles<br />
Die Zahnpolitur mit ihren verschiedenen Produkten, Geräten, deren Vor- und Nachteile, stellt<br />
einen wesentlichen Bestandteil einer guten Zahnreinigung dar. Zudem werden verschiedene<br />
Fluoridierungskonzepte vorgestellt.<br />
Das Perioscope: Einblick in eine neue Welt<br />
Zur Beurteilung der eigenen Arbeit und zur gründlichen Bearbeitung der Wurzeloberfläche<br />
dient das "Perioscope", ein Endoskop, das uns einen Blick auf subgingival gelegene Bereiche<br />
erlaubt, auch ohne chirurgische Maßnahmen. Gezeigt werden Vorgehen und Arbeitsweise<br />
des Gerätes, Grenzen und Möglichkeiten im Einsatz beim subgingivalen Scaling in der Praxis.<br />
Einsatz von Parodontaldiagnostika<br />
Immer wieder erleben wir, dass Patienten auf unsere konservative Parodontitistherapie nicht<br />
mit dem gewünschten Ergebnis antworten. Was ist verantwortlich für sog. "Resttaschen", für<br />
schnellen Knochenabbau trotz guter Mundhygiene? Welche Parodontitistherapie für welchen<br />
Patienten? Parodontaldiagnostika sollen uns helfen, eine adäquate Therapie auch für schwierige<br />
Patientenfälle bereit zu halten.<br />
Mundhygiene im Alter<br />
Die richtige orale Hygiene im Alter rückt immer mehr in den Vordergrund. Immer mehr Menschen<br />
behalten ihre eigenen Zähne bis ins hohe Alter. Die Industrie stellt besonders für diese<br />
Personengruppe entsprechende Mundhygieneartikel zur Verfügung. Ältere und betagte Menschen<br />
bedürfen einer speziell auf ihre Belange zugeschnittenen Therapie. Diese und die dafür<br />
benötigten Hilfsmittel sind Inhalt des abschließenden Themas.<br />

Parodontalchirurgie: wann resektiv, wann regenerativ ?<br />
Wenn regenerativ, welche Methode bringt die größten Vorteile ?<br />
Deep intra-osseous defects represent a major challenge for the clinician. Sites with intra-osseous lesions<br />
have been shown to be at higher risk of disease progression in subjects who had not received systematic<br />
periodontal therapy. Treatments of intrabony defects include scaling and root planing with or without<br />
surgical access flap. When the periodontal debridement is surgically performed, additional osseous resective<br />
therapy and/or reconstructive therapy by means of the application of membranes, biological agents<br />
or grafting biomaterials can be used to correct the bone deformities induced by destructive periodontal<br />
disease. In this respect, reconstructive procedures have been used with varying success during the past<br />
decades to accomplish the restitutio ad integrum of lost attachment apparatus in deep intraosseous<br />
defects. Despite the widely accepted effectiveness of the reconstructive procedures previously documented,<br />
there is a substantial variation in clinical response which may be only in part attributed to the biomaterials/biological<br />
agents adopted. Several traditional reviews have previously reported the additional<br />
effect of the use of different reconstructive procedures in intraosseous defects. Unfortunately, it has been<br />
shown that many traditional, medical reviews are haphazard and biased, often reflecting the opinion of<br />
the review's authors. In contrast, systematic reviews follow explicit, well-documented, scientific methodology<br />
in order to provide a more objective, comprehensive view of the research literature. Since all reconstructive<br />
procedures are time-consuming, technically demanding and financially costly, in view of the high<br />
expense of the reconstructive device/biomaterials and surgical time, systematic reviews are of paramount<br />
importance to assess the efficacy of reconstructive procedures in the treatment of periodontal intraosseous<br />
defects measured against the current standard of surgical periodontal treatment, access flap surgery.<br />
In view of these consideration, the aims of the seminar are:<br />
1) to define when and to what extent an osseous resective approach is indicated to treat an intraosseous<br />
defect;<br />
2) to determine the effect of the use of guided tissue regeneration, grafting procedures or the application<br />
of enamel matrix proteins in addition to conventional open flap debridement (OFD) in the treatment<br />
of deep intraosseous defects. Specifically, the additional efficacy of each reconstructive procedure in comparison<br />
to OFD only procedure will be evaluated by means of clinical and patient-centred outcome parameters<br />
on a short-term and long-term basis.<br />
3) to define which factors (related to the selection of the patient, defect and surgical procedures) may<br />
affect the treatment outcome.<br />
