Aims: Angiotensin (Ang) II activates NF-kB and<br />
thereby induces hypertension-induced target<br />
organ damage. Bcl10 is a member of the CAR-<br />
MA3-Bcl10-MALT1 signalosome complex that<br />
activates IKK-NF-kB signaling. We tested the<br />
role of Bcl10 in the pathogenesis of Ang II-<br />
induced cardiac and renal damage.<br />
Methods: We used male Bcl10 knockout mice<br />
(Bcl-/- ) and wild-type sham controls. The uninephrectomized<br />
mice were given 1% NaCl in<br />
the drinking water and all received Ang II (1.44<br />
mg/kg/d) for 14 days. Blood pressure was measured<br />
by telemetry. To test for inducibility of<br />
arrhythmias, programmed ventricular stimulation<br />
protocols were used. Renal tubular damage<br />
was assessed by measuring neutrophil<br />
gelatinase-associated lipocalin mRNA expression<br />
by qPCR. Albuminuria was measured<br />
before and after the Ang II treatment. Cardiac<br />
weight was measured after Ang II treatment.<br />
Both kidney and heart samples were histologically<br />
examined.<br />
Results: Ang II-treated Bcl10-/- and controls<br />
showed similar mean blood pressures (147±9<br />
vs. 145±4 mmHg). However, Bcl10-/- mice developed<br />
less cardiac hypertrophy, less fibrosis, and<br />
fewer infiltrating macrophages, CD4 and CD8 T<br />
cells compared to controls. Also the number of<br />
reproducible nonsustained ventricular arrhythmias<br />
was decreased in Bcl10-/- animals (67%<br />
vs. 10%). In the kidney, tubular damage was<br />
reduced in Bcl10-deficient mice; histological<br />
analysis revealed less vascular damage, less<br />
interstitial fibrosis and reduced number of infiltrating<br />
inflammatory cells. Nevertheless, albuminuria<br />
was higher in Bcl10-/- than in controls<br />
(789±205 vs. 210±49 µl/day). Glomerular nephrin<br />
expression and the number of the podocytes<br />
were reduced in Bcl10-/- compared to controls.<br />
Conclusion: Thus, Bcl10 promotes enhanced<br />
cardiac, vascular and tubular damage as well<br />
as increased fibrotic and inflammatory processes<br />
in response to Ang II. In contrast, glomerular<br />
Bcl10 deficiency results in increased podocyte<br />
damage in the presence of Ang II leading to increased<br />
albuminuria. Our results demonstrate<br />
that Bcl10 is involved in mediating Ang II pathogenesis<br />
in a cell-specific manner.<br />

Hintergrund: In tierexperimentellen Modellen<br />
wurde eine erhöhten Stickstoffmonoxid (NO)-<br />
Produktion bei Tieren mit (MS+) verglichen<br />
mit Tieren ohne metabolisches Syndrom (MS-)<br />
gezeigt. Die erhöhte basale NO-Produktion<br />
war nicht mit einer erhöhten endothelabhängigen<br />
Vasodilatation assoziiert. Dies wurde<br />
in dieser Studie bei adipösen Patienten (MS+<br />
vs. MS-) mittels in vivo Untersuchung der retinalen<br />
Durchblutung untersucht.<br />
Patienten und Methode: Die retinale kapilläre<br />
Perfusion (RCF) von 67 adipösen Patienten<br />
(31 MS+ und 36 MS-) im Alter von 39± 6Jahren<br />
wurde mittels Scanning Laser Doppler Flowmetry<br />
(SLDF - Heidelberg Engineering /<br />
Deutschland) untersucht. RCF wurde nach<br />
einer Ruhephase von 30 Minuten (baseline),<br />
3 Minuten nach dem Start einer Flickerlicht-<br />
Stimulation und 5 min nach intravenöser Gabe<br />
von 3 mg/kg L-NMMA gemessen. Die visuelle<br />
Stimulation (Luminanzflicker, 10Hz) kann zur<br />
Bewertung der endothelabhängigen Vasodilatation<br />
und die Gabe von L-NMMA, ein NO-<br />
Synthase-Inhibitor, zur Bewertung der basalen<br />
NO-Aktivität genutzt werden.<br />
Ergebnisse: MS+ zeigten einen höheren Ruhe-<br />
RCF (350±75AU vs. 314±66AU, p=0.059) als MS-.<br />
Der RCF nach Flickerstimulation erhöhte <strong>sich</strong><br />
signifikant sowohl bei MS+ (9.5±15.8, p< 0.001)<br />
als auch bei MS-: (8.3±16.0, p=0.014) ohne<br />
signifikante Differenz zwischen MS+ und MS-<br />
(p=0.68). Nach L-NMMA-Infusion zeigte <strong>sich</strong><br />
eine signifikante Reduktion des RCF in beiden<br />
Gruppen mit einen signifikanten Unterschied<br />
zwischen den Gruppen (MS+:-7.9±10% versus<br />
MS-: -2.4 ±9%, p=0.044). Weiterhin korrelierte<br />
der Ruhe-RCF und die Abnahme des RCF<br />
nach L-NMMA in beiden Gruppen signifikant<br />
(r=-0.34, p=0.005) und in einzelnen Gruppen<br />
tendenziell (MS+: r=-0.30, p=0.11, MS-:r=-0.29,<br />
p=0.09).<br />
Schlussfolgerungen: Metabolische Patienten<br />
haben verglichen mit Patienten ohne metabolisches<br />
Syndrom eine erhöhte retinale Ruheperfusion<br />
und eine verstärkte Abnahme des<br />
RCF auf L-NMMA, hinweisend auf eine erhöhte<br />
basale NO-Aktivität. Demgegenüber ist die endothelabhängige<br />
Vasodilatation zwischen MS+<br />
und MS- nicht unterschiedlich.<br />

Background: Systemic hypertension is associated<br />
with retinal arteriolar narrowing and a<br />
lower retinal arteriolar-to-venular diameter<br />
ratio (AVR) has been shown to predict the risk<br />
of hypertension. We investigated the effects of<br />
different blood pressure levels on the retinal<br />
microcirculation and the association with<br />
systemic arterial stiffness, renal and cardiovascular<br />
risk factors (CVRF).<br />
Methods: The study included 80 healthy male<br />
subjects aged 18-40 years with a history of high<br />
normal office blood pressures in the past two<br />
years. Office (OBP), home-based (HBP) and<br />
central blood pressures (CBP) were taken and<br />
categorized into optimal/normal (o/n), highnormal<br />
(h-n) and high blood pressure levels.<br />
Retinal vessel diameters were digitally assessed<br />
using a static retinal vessel analyzer.<br />
Arterial stiffness and central blood pressures<br />
were measured by pulse wave analysis using a<br />
sphygmograph.<br />
Results: Taking OBP, 29% of participants had<br />
o/n blood pressure (mmHg: 122.2±5.7/76.7±5.2),<br />
