Release of liability: As parent or legal guardian of the child named above, I certify that my child has permission to attend<br />
and participate in the <strong>Tybee</strong> Children’s Fishing Tournament to be held on Saturday, June 9, 2007 at Chimney Creek Fishing<br />
Camp, beginning at 8:30 A.M. and ending at approximately 4PM. By signing this form, I understand that I am waiving all<br />
right to claims and releasing all other participants, the hosts, sponsors, and other officials involved from all liability for<br />
injuries or damages that may occur as a result of this tournament, or my child’s participation in same. I also grant the<br />
unconditional right to use the name, voice and photographic image of my child in connection with any of the audio/video<br />
productions, articles, or press releases but not as a paid endorsement.<br />

RULES<br />
1. All Children must be in the company of an adult at all times<br />
2. All children must wear life jackets at all times<br />
3. Child must manage the rod at all times but adults may net the fish<br />
4. IGFA and State Rules will be followed. No toadfish, catfish or shark<br />
5. Registration forms & $3.00 registration fee must be turned in to the<br />
Gift Shack at The Crab Shack by May 24 th . After that date, registration is $5.00<br />
6. Each child is limited to winning one (1) major prize or award<br />

8:30 A.M. Captain’s Meeting 2:00 Weigh-in Ends<br />
9:30 A.M. Fishing Starts 2:30 Awards & Door Prizes<br />
1:30 P.M. Fishing Ends FOOD & FUN!!!!!!!!!!!<br />
Open to Children ages 0 - 13<br />
Dock Fishing: Pending approval to rebuild from State & Federal Agencies, Chimney Creek’s docks are<br />
closed and may or may not be open for the tournament. The restaurant docks are open (but fish are not<br />
there to be caught). Prizes will be given for dock fishing as well as boat fishing in age groups 0 – 6 and<br />
7 – 13.<br />
