CHAPTER III BASIC <strong>DATA</strong> <strong>ENTRY</strong> INSTRUCTIONS<br />

c. If age is not stated and date of birth and date of death are the<br />

same consider decedent under 1 day old, leave the Age<br />

(Number of Units) field on the screen blank and type HOURS in<br />

the UNIT field.<br />

d. If Age and the Date of Birth entries on the certificate are blank,<br />

leave the Age (Number of Units) field blank on the screen. Enter<br />

the UNIT as indicated on the certificate or type UNKNOWN if<br />

units are not indicated.<br />

e. If age cannot be determined from the certificate, but entries of<br />

"newborn," "infant," etc., indicate an age of less than 1 year old,<br />

leave the Age (Number of Units) field on the screen blank and<br />

type HOURS in the UNIT field.<br />

f. Disregard fractions reported in the number of units for age on the<br />

certificate.<br />

g. If the number of units on the certificate is reported with<br />

"approximately," "about," etc., enter the number only.<br />

Part I, Cause of Death - Type in the cause of death exactly as it<br />

appears on the death certificate. Do not make any alterations. If<br />

any word or words in the Cause of Death are illegible, type<br />

ILLEGIBLE in place of the illegible words. All other legible words<br />

should still be entered.<br />

Appendix D contains a list of symbols which cannot be easily<br />

entered using a PC keyboard. Refer to this appendix to enter the<br />

correct text.<br />
