CHAPTER III BASIC <strong>DATA</strong> <strong>ENTRY</strong> INSTRUCTIONS<br />

Certifiers may include abbreviations for medical conditions. It is<br />

correct to enter these abbreviations as they are reported. In<br />

addition, <strong>SuperMICAR</strong> users may use abbreviations given in<br />

Appendix E. These are the ONLY acceptable abbreviations that<br />

can be used by the coder to shorten entries. When one of these<br />

abbreviations is used, a semi-colon (;) must be entered after the<br />

abbreviation if it is a complete condition. Enter abbreviations that<br />

are not on this list only when this information is reported on the<br />

record by the certifier.<br />

Read the reported conditions carefully and accurately. Use<br />

abbreviations only when certain that it is correct to do so. For<br />

example, HEM is an acceptable abbreviation for Hemorrhage but<br />

never for Hemorrhagic. If in doubt, it is best to enter full text.<br />

Abbreviations should be used to conserve time and increase<br />

production, and should not bog down work and prove more time<br />

consuming than full text.<br />

Duration - Type in the interval between the onset of the condition<br />

and death exactly as it appears on the death certificate. Do not<br />

make any alterations. Example: 3-4 WEEKS, 2 1/2 MONTHS, etc.<br />

a. Duration may be reported using units of time (YEARS,<br />

MONTHS, WEEKS, DAYS, HOURS, etc.) or with words such as<br />

BRIEF, CHRONIC, CONGENITAL, MOMENTS, NEWBORN, etc.<br />

b. Some of the special symbols listed under Cause of Death, Part I<br />

also apply to the Duration field. See Special Symbols in<br />

<strong>SuperMICAR</strong>.<br />

c. If a number other than a "1" or a "2" appears with a degree<br />

symbol (Example: 8°) in the Duration, ignore the number and<br />

symbol. Do not enter the number and symbol.<br />

Part II, Cause of Death - Type in any other significant conditions<br />

that are listed in the Cause of Death section of the death certificate<br />

in the order as reported.<br />
