All records are checked for completeness and accuracy, and then given a <strong>State</strong> file number before they are encoded and<br />

entered into the <strong>State</strong>’s databases. Birth and death records are kept on permanent file for certification use and amendment when<br />

necessary by the public and various government agencies. Marriage, divorce, and abortion records are not maintained as<br />

separate records after entry into the <strong>State</strong>’s database.<br />

When a certificate is found to be inaccurate or incomplete, personnel follow up with the providing entity in accordance with<br />

NRS 440.150 to make the necessary corrections and/or improvements to make the record complete and satisfactory. So, it<br />

should be noted that the quality of data presented in this report is directly related to the completeness and accuracy of the<br />

information contained on the certificates.<br />

This report summarizes data compiled from original birth, death, stillbirth and induced abortion, marriage and divorce<br />

records filed with the Office of Vital Records as discussed in NRS 440. Standard forms recommended by the National Center<br />

for <strong>Health</strong> Statistics are used for all records, with the exception of marriage and divorce.<br />
