APPENDIX 1A: INFORMED CONSENT FORM<br />

Good Morning, /afternoon/evening, my name is Ms Lydia Hangulu. I am a masters student in<br />

the School <strong>of</strong> Psychology, <strong>University</strong> <strong>of</strong> Kwa-Zulu <strong>Natal</strong>, 4041, Durban, South Africa. I am<br />

conducting research on infection control practices in home-based care organisations for<br />

people living with HIV/AIDS in Durban, South Africa, I would speak to you only if you are<br />

willing to participate in this research.<br />

This discussion will take about 40 minutes-1 hour. I will ask you to talk about your<br />

experiences regarding infection control practices in home-based care. I will need your<br />

permission to use an audio-tape recorder to capture our discussion.<br />

All information that you give will be kept confidential. The information collected will be<br />

stored in my supervisor’s <strong>of</strong>fice and only research assistants working with me on this project<br />

will have access to it. Information will be used for research purposes alone and raw data will<br />

be destroyed as soon as the study is completely over. Also, we will not use your actual name<br />

or designation in reporting the findings <strong>of</strong> the study but will use disguised name to make sure<br />

that no one links the information you have given us to you.<br />

You will not be given any monetary payments for participating in the study but your<br />

department/ organizations/ community/ the government will benefit from this study<br />

immensely. The results will help us to understand the challenges encountered by your<br />

organisation regarding infection control practices.<br />

Your participation in this study is voluntary. If you agree to take part in the study, we will ask<br />

you to sign a form as an indication that we did not force you to participate in the study. Please<br />

note that you will not be at any disadvantage if you choose not to participate in the study.<br />

You may also refuse to answer particular questions if you don’t feel comfortable answering<br />

them. You may also end the discussion at anytime if you feel uncomfortable with the<br />

interview. In case you want to withdraw information given after the interview, you can call<br />
