<strong>The</strong> Diagnosis <strong>of</strong> Salt Poison<strong>in</strong>g Lead<strong>in</strong>g to Hypematraemia <strong>in</strong> Children – September 2009<br />

In a child <strong>with</strong> <strong>hypernatraemia</strong> a ur<strong>in</strong>e osmolality can assist <strong>in</strong> exclud<strong>in</strong>g diabetes <strong>in</strong>sipidus [Grade<br />

D]. It is best <strong>in</strong>terpreted <strong>in</strong> conjunction <strong>with</strong> a simultaneous serum osmolality.<br />

Differential <strong>diagnosis</strong> <strong>of</strong> salt poison<strong>in</strong>g<br />

As well as the causes <strong>of</strong> excessive sodium <strong>in</strong>take there are a number <strong>of</strong> other causes <strong>of</strong><br />

significant <strong>hypernatraemia</strong>, all due to water depletion. Although <strong>hypernatraemia</strong> may occur<br />

<strong>with</strong> m<strong>in</strong>eralocorticoid excess, this is mild and it is rarely, if ever, severe enough to constitute a<br />

cl<strong>in</strong>ical problem.<br />

Lack <strong>of</strong> water <strong>in</strong>take:<br />

1. Inadequate breast milk result<strong>in</strong>g <strong>in</strong> <strong>in</strong>adequate water <strong>in</strong>take<br />

2. Unconscious or mentally impaired (described <strong>in</strong> adults)<br />

3. Essential <strong>hypernatraemia</strong> <strong>with</strong>out structural abnormality <strong>of</strong> the hypothalamus<br />

4. Adipsia secondary to structural abnormalities <strong>in</strong>volv<strong>in</strong>g the hypothalamus<br />

5. Deliberate water restriction<br />

6. Lost <strong>in</strong> the desert or at sea <strong>with</strong> lack <strong>of</strong> available water comb<strong>in</strong>ed <strong>with</strong> <strong>in</strong>creased evaporative<br />

water loss.<br />

Case reports <strong>of</strong> osmoreceptor dysfunction are all <strong>in</strong> association <strong>with</strong> other endocr<strong>in</strong>e or structural<br />

bra<strong>in</strong> abnormalities. <strong>The</strong>re are no case reports <strong>of</strong> otherwise normal <strong>children</strong> present<strong>in</strong>g <strong>with</strong><br />

isolated <strong>hypernatraemia</strong> be<strong>in</strong>g due to abnormal osmostats [Grade C].<br />

Increased water loss<br />

Increased renal loss<br />

1. Central diabetes <strong>in</strong>sipidus<br />

2. Nephrogenic diabetes <strong>in</strong>sipidus<br />

3. Renal medullary damage<br />

Increased gastro<strong>in</strong>test<strong>in</strong>al loss<br />

Hypernatraemic dehydration secondary to diarrhoea<br />
