The strategy will support system <strong>and</strong> service development as well as the implementation of models that<br />

can achieve:<br />

• improved access to general practice services, particularly for those from disadvantaged communities in<br />

areas of high need<br />

• provision of high quality multidisciplinary care <strong>and</strong> care coordination for those with chronic <strong>and</strong><br />

complex conditions.<br />

Refer to <strong>funding</strong> <strong>guidelines</strong> to be developed<br />

The strategy will focus on improving access to general practice services for disadvantaged communities in<br />

areas of high need <strong>and</strong> the provision of coordinated, multidisciplinary care to those with chronic <strong>and</strong><br />

complex conditions.<br />

Agency discretion should be used in determining whether fees are charged for GP services.<br />

To achieve health service integration across the health care continuum, it is expected that the practice of<br />

service coordination is embedded into service delivery.<br />

Quality health promotion practice (as outlined in the Integrated <strong>Health</strong> Promotion Framework) is to be<br />

embedded within health promotion planning, implementation <strong>and</strong> evaluation.
