PCP development work to be introduced from June 2009<br />

includes: improved Integrated <strong>Health</strong> Promotion<br />

Performance Measures for all <strong>Primary</strong> <strong>Health</strong> programs<br />

funded for health promotion; <strong>and</strong> a revised suite of the<br />

Service Coordination Tool Templates as part of a continuous<br />

improvement approach to service coordination.<br />

I look forward to working with the sector <strong>and</strong> relevant<br />

government program partners over the coming year to<br />

promote the delivery of quality primary health care services<br />

in Victoria. I am committed to supporting the development of<br />

more coordinated, timely, high quality services which<br />

improve health outcomes for all Victorians, particularly those<br />

at most disadvantage.<br />
