[CDIFF] Clostridium Difficile Toxins<br />

SPECIMEN REQUIRED: Loose stool in a leakproof specimen cup<br />

without transport media or preservatives. If unable to submit<br />

immediately to the <strong>OU</strong>MC Lab, refrigerate (4 degrees C) for<br />

no more than 72 hours. Rectal swabs and formed stool<br />

specimens will be rejected.<br />

TUBE OR CONTAINER: Screw cap container.<br />

SPECIAL HANDLING: <strong>Test</strong> will not be performed on formed stool or<br />

swabs, on patients less than 1 year of age, or on patients<br />

with a previous positive Toxin B assay in the last 30 days.<br />

TEST AVAILABILITY: Daily<br />

METHODOLOGY: PCR (Polymerase Chain Reaction)<br />

CLINICAL INFORMATION: Aid in diagnosis of antibiotic related<br />

colitis. The C. difficile PCR has a reported negative<br />

predictive value of greater than 98%. Therefore, the test<br />

will only be performed once every 7 days when C. difficileassociated<br />

colitis is strongly suspected. Otherwise, alternate<br />

causes of diarrhea should be sought.<br />

SPECIMEN REQUIRED: Loose stool in a leakproof specimen cup<br />

without transport media or preservatives. If unable to submit<br />

immediately to the <strong>OU</strong>MC Lab, refrigerate (4 degrees C) for<br />

no more than 72 hours or freeze (-10 C or lower). Rectal swabs<br />

and formed stool specimens will be rejected.<br />

METHODOLOGY: Enzyme immunoassay (EIA)<br />

CLINICAL INFORMATION: Aid in diagnosis of antibiotic related<br />

colitis. The C. difficile Ag-EIA has a reported negative<br />

predictive value of greater than 99%.1 Therefore, the test<br />

should only be repeated when C. difficile-associated colitis<br />

is strongly suspected. Otherwise, alternate causes of diarrhea<br />

should be sought.<br />

Stools positive for Clostridium difficile antigen will be<br />

tested for Toxin B to confirm the presence of a toxin-producing<br />

strain of C. difficile at an additional charge.<br />
