Dispatch centre received emergency call:<br />

Accident of 2 vehicles, number of injured unknown, fire brigade and police on their way.<br />

Your task:<br />

assess scene and correct work management on site<br />

examine, secure, treat and prepare patients for transport<br />

triage and routing of patients<br />

Local situation:<br />

Nearest hospital is 20 km by ground transport with surgery, internal medicine (neurologists on duty nonstop),gynecology and obstetric and biochemistry.<br />

Higher level hospital 42 km by ground transport, dept. as A + ED, anaesthesia and General Intensive Care, ENT, CT, neurologic dept. with ICU, psychiatry, infectious diseases and pediatric dept.<br />

Specialized centre 55 km by ground. Depts. as B + traumacentre, burn unit, cardiocentre, stroke unit, NMR.<br />

Helicopter rescue not available.<br />

Situation:<br />

1. vehicle with minimal damage - 1 person - driver not injured<br />

2. older vehicle minor deformation of interier - total of 5 persons: 2 injured inside (non trapped), 2 outside, 5th is sitting in the undamaged car. Police present, fire brigade arrives.<br />

Injuries:<br />

Driver of vehicle 2, sitting in dirver seat, fastened seatbelts, desoriented, complains about pain on chest, abdomen, left hip joint with restriced movability, GCS 4-4-6, BP 140/100, capillary refill > 2 sec.,<br />

HR 110/min., RR 15/min., SpO2 90 %<br />

passenger back right, seatbelts not fastened (child - teenager), complains about big pain on right femur: fracture of right femur, flesh wound on head, GCS 4-4-6 , capillary refill > 2 sec., BP 100/60, HR<br />

120/min., RR 20/min., SpO2 85 %<br />

Walking passenger back left (child - teenaer), seatbelts not fastened. Complains about small headache, minor flesh wound of head and upper extremities, GCS 4-5-6, pulse on aa. Radialis full, capillary<br />

refill < 2 sec., BP 110/70, HR 110/min., RR 15/min., SpO2 98 %.<br />

Passenger back mid (child - teenager), seatbelts not fastened, in this time in other vehicle: complains about small headache, flesh wound on front of head - treated, pulse na aa.radialis full, otherwise no<br />

signs of injury, GCS 4-4-6, capillary refill < 2 sec., BP 100/70, HR 120/min., RR 16/min., SpO2 95 %<br />

Walking co-driver, fastened seatbelts, in this time outside, alone, tries to look like a by-walker, on repeated questioning answers that he was in the car but asks the crew to prefer other patients, denies any<br />

injuries, after repeated questioning announces chest pain (left side), exam by palpation painfull left hemithorax - fracture of ribs, otherwise no signs of injury, GCS 4-5-6, capillary refill < 2 sec., BP 130/90,<br />

HR 100/min., RR 12/min., SpO2 97 %.<br />

Vehicle 1 driver: no injuries, denies any complications, GCS 4-5-6, BP 150/100, HR 105, SpO2 95 %<br />

Point of task:<br />

To check basic rescuer experience at car accident with bigger number of injured and basic co-operation with Dispatch Center. Main point of task is correct triage.<br />

pelvis, upper limbs, lower limbs, capillary return,<br />

GCS,neurological status - movability and sensitivity<br />

of limbs, HR, BP, breathing, SpO2, glycaemia at F2,<br />

F1, F4 ,<br />

At triage in car analgesia and securing before<br />

extrication.<br />

Gentle extrication F1 and F2 with the use of<br />

extricational equipment or in co-operation with fire<br />

brigade under lead of crew member (Kendrik's vest,<br />

scoop) monitoring.<br />

Transport in at least 3 ambulance cars, triage positive<br />

F1, F2 routing to traumacentre.<br />

Subjective rating of player.<br />
