Local situation:<br />
Nearest hospital is 20 km by ground transport with surgery, internal medicine (neurologists on duty nonstop),gynecology and obstetric and biochemistry.<br />
Regional hospital 42 km by ground transport, dept. as A + ED, anaesthesia and General Intensive Care, ENT, CT, neurologic dept. with ICU, psychiatry, infectious diseases and pediatric dept.<br />
Specialized centre 55 km by ground. Depts. as B + traumacentre, burn unit, cardiocentre, stroke unit, NMR.<br />
Helicopter rescue available at 15 mins.<br />
Situation on the scene:<br />
Healthy man 50 years after night shift at railway. Instructions given by spouse by phone: he sounds "strange", reluctant, dysarthric. suddenly finished phonecall. At Ambulance arrival somnolent<br />
man open the door, no complains. Reacts according the crew - agressive to aggresive approach, polite to polite crew.<br />
Objective: healthy 50 years old man with normal pfysical fiundings, smoker, otherwise negative PH.<br />

HYPOTHESIS: more than 1/2 crew with physician do not examinate in detail. More than 1/4 of paramedics crew do not provide physical examination.<br />
