matters also may be sent via e-mail at consumer@mid.state.ms.us . Be sure to include your mailing address and<br />
telephone number with any correspondence directed to MID. Complaints filed by e-mail may be returned with<br />
instructions for you to resubmit the claim in writing with your signature in order for MID to be able to take specific<br />
corrective action that may be required.<br />
What should you include in your complaint?<br />
You must include the following information in order for the Mississippi Insurance Department to be able to properly<br />
process your complaint:<br />
· your name and your relationship to the insured.<br />
· telephone number where you can be reached during the day.<br />
· name <strong>of</strong> insured.<br />
· insured's address name, address, city, zip code, and phone number.<br />
· the name <strong>of</strong> the insurance company with which the insured is having a problem.<br />
· address <strong>of</strong> the insurance company with which the insured is having a problem.<br />
· type <strong>of</strong> insurance, policy number, claim number, and date <strong>of</strong> loss.<br />
What are the alternative ways <strong>of</strong> filing a complaint?<br />
There are two alternative ways <strong>of</strong> filing a complaint:<br />
1. Visit the National Association <strong>of</strong> Insurance Commissioners' (NAIC) Consumer Information Source<br />
(www.naic.org/cis/) site and click on "File a Consumer Complaint" to file your complaint via email with the<br />
Mississippi Department <strong>of</strong> Insurance.<br />
2. Download MID's Complaint Reporting Form (www.doi.state.ms/US/consumer/midcmfm.pdf) (Adobe Acrobat<br />
Reader required), complete, then print and mail to MID at P.O. Box 79, Jackson, MS 39205. Once they receive your<br />
correspondence, it will be assigned to one <strong>of</strong> MID's investigators, who will review it and take the necessary steps to<br />
resolve the matter. Allow MID 20 working days to contact the company or agency that is the subject <strong>of</strong> your<br />
complaint. They will notify you in writing <strong>of</strong> their findings.<br />
