How Do I Replace My Social Security Card?<br />

In order to replace your Social Security card, you must complete form SS5 ("Application for a Social Security<br />

Card"). This form is available at your nearest open Social Security <strong>of</strong>fice or outreach site. You can also obtain form<br />

SS5 by calling 18007721213 or downloading form SS5 at http://www.ssa.gov.<br />

You should go to the nearest open Social Security <strong>of</strong>fice to apply for your replacement card in person. They will give<br />

you a print out <strong>of</strong> your card at the local <strong>of</strong>fice.<br />

You will be required to show proper identification (ID) when applying for a replacement card. Proper identification<br />

can include a driver's license, marriage or divorce records, military records, adoption records, life insurance policy,<br />

health insurance card, or school ID card. The Social Security Administration will not accept birth certificates or<br />

hospital birth records as evidence <strong>of</strong> identity.<br />

How Do I Report A Death To The Social Security Administration?<br />

You can report a death to the Social Security Administration by calling 18007721213 and speaking to a service<br />

representative between the hours <strong>of</strong> 7 a.m. and 7 p.m. on any business day. When you call, make sure you have the<br />

deceased person's Social Security number handy. If you are deaf or hard <strong>of</strong> hearing, call 18003250778 between 7 a.m.<br />

and 7 p.m. on any business day.<br />

If you are getting benefits on your spouse's record when he or she dies, the Social Security Administration will<br />

change your payments to Survivor's benefits. If you are getting benefits on your own record, you can apply for<br />

Survivor's benefits. Call or visit an open Social Security <strong>of</strong>fice to find out whether you are eligible for money as a<br />

widow or widower.<br />

Current benefits for children will automatically change to Survivor's benefits after the death is reported to the Social<br />

Security Administration.<br />

Compulsory education law requires that school age children be enrolled in some type <strong>of</strong> <strong>of</strong>ficially recognized school<br />

to relocate and, as a result, to place their children temporarily or permanently in a different school system or setting,<br />

<strong>of</strong>ten in the middle <strong>of</strong> a school term. In the face <strong>of</strong> <strong>Hurricane</strong> <strong>Katrina</strong>, most states have temporarily waived residency<br />

or immunization requirements for school age children.<br />
